温针隔姜灸膝周七穴对膝骨关节炎患者临床疗效及IL-1、滑膜厚度的影响

Indirect Ginger Moxibustion with Warm Needle at Seven Acupoints around the Knee on the Clinical Symptoms,Serum Interleukin-1 and Synovial Thickness in Patients with KOA

目的比较温针隔姜灸和玻璃酸钠关节腔注射治疗对膝骨关节炎(KOA)的疗效。方法将64例KOA患者随机分为两组各32例。试验组予温针隔姜灸治疗,对照组予玻璃酸钠膝关节腔注射治疗,比较两组治疗前、治疗2周后、治疗5周后膝关节功能评分、治疗前后关节炎症指标血清白细胞介素-1(IL-1)及滑膜厚度、治疗结束后整体疗效情况及患者满意度。结果两种治疗方法在2~5周内都能对KOA患者的膝关节功能起到较好的疗效,而温针隔姜灸在较长的治疗时间中,较玻璃酸钠膝关节腔注射的方法对KOA患者膝关节功能改善的优势才能得以体现(P<0.05),且温针隔姜灸组的患者对治疗的美国西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分高于对照组(P<0.05)。结论温针隔姜灸膝周七穴能够降低血清IL-1与滑膜厚度,改善KOA患者临床症状。

Objective:To observe the comparative efficacy of indirect ginger moxibustion with warm needle and sodium vitreous acid joint cavity injection on knee osteoarthritis(KOA).Methods:Sixty-four patients with KOA were randomly divided into a test group(indirect ginger moxibustion with warm needle)and a control group(sodi-um vitreous acid knee cavity injection),32 patients in each group.The two groups were compared in terms of knee function scores before treatment,after 2 weeks of treatment,and after 5 weeks of treatment,joint inflamma-tion indexes(serum interleukin 1(IL-1)and synovial thickness before and after treatment,overall efficacy at the end of treatment,and patient satisfaction.Results:Both treatments had better efficacy on knee function in KOA patients within 2~5 weeks(P<0.01),while the advantage of indirect ginger moxibustion with warm needle over so-dium glacial knee cavity injection on the improvement of knee function in KOA patients could only be realized in the longer treatment time(P<0.05),and the patients in the indirect ginger moxibustion with warm needle group had a higher WOMAC score than that in the control group(P<0.05).Conclusion:Indirect ginger moxibustion with warm needle at seven acupoints around the knee is able to reduce serum IL-1 and synovial thickness and im-prove clinical symptoms in KOA patients.
