钠泵α1亚单位基因在盐敏感性高血压中的作用

摘要 目的 探讨钠泵α1亚单位基因在盐敏感性高血压中的作用,为探明盐敏感性高血压的发病机制及可能的治疗靶点提供思路。方法 选取原发性高血压Ⅰ、Ⅱ期患者,经限盐试验筛选盐敏感患者与非盐敏感患者各27例,选择血压正常的27例健康体检者作为对照组,比较三组之间的一般人口学资料、血生化、24 h尿电解质含量及钠泵α1亚单位基因的mRNA和蛋白表达水平等指标的差异。结果 盐敏感组及非盐敏感组的24 h收缩压、24 h舒张压及平均动脉压均无显著差异,但盐敏感组夜间收缩压及舒张压下降百分比显著低于非盐敏感组。盐敏感组的24 h尿钠含量显著高于非盐敏感组,三组患者的24 h尿钠钾代谢比存在显著差异,盐敏感组的尿钠钾排泄比显著高于对照组。盐敏感组和对照组的醛固酮肾素比值有统计学差异。盐敏感组的钠泵α1亚单位基因mRNA相对表达量低于对照组。盐敏感组、非盐敏感组钠泵α亚单位基因蛋白相对表达量低于对照组。结论 盐敏感高血压钠泵α1亚单位基因表达受限,其表达水平可能与盐敏感高血压的疾病进展紧密相关。 Objective To explore the role of sodium pumpα1 subunit gene in salt-sensitive hypertension,and provide ideas for exploring the pathogenesis and potential therapeutic targets of salt sensitive hypertension.Method 27 cases of salt sensitive(SS)and 27 cases of salt resistant(SR)were screened by salt test from patients with stage 1 and 2 hypertension,while 27 healthy subjects with normal blood pressure were chosen as the control group.The general demographic data,blood biochemistry,24 h urine electrolyte content,mRNA and protein expression levels of sodium pumpα1 subunit gene were compared in three groups.Results No significance of 24 h systolic blood pressure,diastolic blood pressure as well as mean arterial pressure was found between SS and SR,while the percentage of SBP and DBP decreased at night in SS was significantly higher than in SR.24 h urine sodium of SS was significantly higher than in SR.Meanwhile,24 h urine sodium-potassium metabolism ratio of three groups had statistical difference,and aldosterone renin ratio of SS was higher than in control group.There was a statistical difference in the aldosterone renin ratio between the salt sensitive group and the control group.Compared with the control group,the expression of sodium pumpα1-subunit gene mRNA expression was lower in SS.Sodium pumpα1-subunit gene protein expression in SS and SR was lower than that in the control group.Conclusion Sodium pumpα1 subunit gene expression is limited in salt-sensitive hypertension,which may be closely related to the disease progression of salt-sensitive hypertension.
