广东地区下焦湿热型石淋患者体质量指数与胱抑素C的相关性分析

Analysis of Correlation Between Body Mass Index and Cystatin C in Patients with Urolithiasis of Damp Heat in Lower Energizer Type from Guangdong Area

【目的】分析广东地区下焦湿热型石淋(泌尿系结石)患者体质量指数(BMI)与血清胱抑素C之间的相关性,探讨BMI升高对下焦湿热型石淋患者胱抑素C的影响。【方法】选取2020年9月至2022年9月在广州中医药大学第一附属医院门诊及住院部就诊的下焦湿热型石淋患者,共纳入200例。通过医院病历系统收集符合纳入标准的200例患者的一般资料及临床资料,通过医院检验系统收集患者就诊时(术前)血清胱抑素C、β2微球蛋白、尿素氮、肌酐、尿酸、肾小球滤过率及血压等数据,按照BMI数值分为正常组(18.5 kg/m^(2)≤BMI<24 kg/m^(2))60例、超重组(24 kg/m^(2)≤BMI<27 kg/m^(2))80例、肥胖组(BMI≥27 kg/m^(2))60例,比较各组间血压及血液生化指标之间的差异,分析广东地区下焦湿热型石淋患者BMI与血清胱抑素C及血尿酸的相关性。【结果】(1)在广东地区男性下焦湿热型石淋患者中,其收缩压、舒张压、胱抑素C、尿素氮、尿酸数值均为肥胖组>超重组>正常组,差异均有统计学意义(P<0.05或P<0.01)。(2)在广东地区女性下焦湿热型石淋患者中,其血清胱抑素C、血尿酸数值均为肥胖组>超重组>正常组,差异均有统计学意义(P<0.01)。(3)经Spearman相关分析,广东地区下焦湿热型石淋患者的BMI与血清胱抑素C高度相关(r=0.753,P<0.001),与血尿酸中度相关(r=0.446,P<0.001)。【结论】广东地区下焦湿热型石淋患者的BMI与血清胱抑素C具有显著相关性;提示该地区的石淋患者应积极控制体质量,注意合理饮食和适度运动,并给予早期临床干预,以预防肾损害的发生。

Objective To analyze the correlation between body mass index(BMI)and serum cystatin C in patients with urolithiasis of damp heat in lower energizer type form Guangdong area,and to investigate the effect of BMI increase on cystatin C in the patients.Methods A total of 200 patients with urolithiasis of damp heat in lower energizer type who attended the outpatient and inpatient departments of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from September 2020 to September 2022 were included.The general information and clinical data of the 200 qualified patients were collected through the hospital medical record system,and the data of serum cystatin C,β2-microglobulin,blood urea nitrogen,creatinine,uric acid,glomerular filtration rate and blood pressure at the visit or before operation were collected through the hospital laboratory examination system.With reference to the BMI,the patients were divided into normal group(18.5 kg/m^(2)≤BMI<24 kg/m^(2),60 cases),overweight group(24 kg/m^(2)≤BMI<27 kg/m^(2),80 cases)and obese group(BMI≥27 kg/m^(2),60 cases).The differences in blood pressure and blood biochemical parameters between the three groups were compared,and the correlation between BMI and serum cystatin C and blood uric acid was analyzed in the patients with urolithiasis of damp heat in lower energizer type from Guangdong area.Results(1)Among the male patients with urolithiasis of damp heat in lower energizer type from Guangdong area,their systolic blood pressure,diastolic blood pressure,cystatin C,urea nitrogen and uric acid values were all in the sequence of obese group>overweight group>normal group,and the difference was statistically significant(P<0.05 or P<0.01).(2)In the female patients with urolithiasis of damp heat in lower energizer type in Guangdong area,their serum cystatin C and blood uric acid values were all in the sequence of obese group>overweight group>normal group,and the differences were all statistically significant(P<0.01).(3)The Spearman correlation analysis showed that BMI was highly correlated with serum cystatin C(r=0.753,P<0.001)and was moderately correlated with blood uric acid(r=0.446,P<0.001)patients with urolithiasis of damp-heat in the lower energizer type in Guangdong.Conclusion The BMI of patients with urolithiasis of damp heat in lower energizer type from Guangdong area is significantly correlated with serum cystatin C,and the results indicated that the patients with urolithiasis in this region should actively control body mass,take reasonable diet and moderate exercise,and seek early clinical intervention to prevent the development of renal damage.
