基于数据挖掘技术探讨针灸治疗肋间神经痛的选穴规律

Exploration of Acupoint Selection Rules of Acupuncture and Moxibustion in the Treatment of Intercostals Neuralgia Based on the Data Mining Technology

【目的】基于数据挖掘技术探讨针灸治疗肋间神经痛的选穴规律。【方法】计算机检索中国期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学文献(CBM)数据库中建库至2022年9月12日的有关针灸治疗肋间神经痛的临床随机对照试验文献,采用SPSS 26.0统计软件对针灸治疗方法、腧穴频次、归经进行描述性分析,运用SPSS Modeler 18.0软件中的Apriori算法对高频腧穴进行关联规则分析并构建关联网络图。【结果】最终纳入274篇文献,共133首选穴处方,涉及腧穴70个,累计使用频次936次:①腧穴使用频次前3位为阿是穴、夹脊穴、支沟穴;②归经频次最高者为手少阳三焦经,其次是足少阳胆经、足太阴脾经;③核心腧穴处方为“阿是”“夹脊”“支沟”,核心穴对为“阿是-夹脊”。【结论】针灸治疗肋间神经痛的腧穴处方以远近搭配、整体-局部并重为特色,注重针灸腧穴的中医理论与现代神经解剖相结合,以达行气活血、疏泄少阳、通络止痛之功。

Objective To explore the acupoint selection rules of acupuncture and moxibustion in treating intercostals neuralgia based on the data mining technology.Methods The literature on clinical randomized controlled trials of acupuncture and moxibustion for intercostal neuralgia from the CNKI,Wanfang,VIP,and CBM database built up to September 12,2022 was screened by computer.SPSS 26.0 statistical software was used to conduct descriptive analysis on acupuncture treatment methods,frequency of acupoints and meridian tropism.Apriori algorithm in SPSS Modeler 18.0 software was used to analyze association rules on high frequency acupoints and construct association network diagram.Results A total of 133 preferred acupoint prescriptions,involving 70 acupoints,were included in 274 articles,with a cumulative frequency of 936 times:(1)the top 3 acupoints in terms of frequency of use were the Ashi points,Jiaji points(EX-B2),Zhigou(SJ6)point;(2)the highest frequency of meridian tropism was to the hand shaoyang triple energizer meridian,followed by the foot shaoyang gallbladder meridian and the foot taiyin spleen meridian;(3)the core acupoint prescriptions were“Ashi points”“Jiaji points”“Zhigou point”,and the core acupoint pair was“Ashi points-Jiaji points”.Conclusion The therapy of acupuncture and moxibustion for the treatment of intercostal neuralgia are characterized by a combination of distant and near acupoints and a holistic-local approach,focusing on the combination of the traditonal Chinese medicine theory of acupoints and modern neuroanatomy,in order to achieve the function of moving qi and activating blood,soothing shaoyang,and unblocking the collaterals and relieving pain.
