股骨近端防旋髓内钉内固定术对股骨粗隆间骨折患者功能康复及并发症的影响

Effect of Proximal Femoral Nail Anti-rotation Internal Fixation on Functional Rehabilitation and Complications in Patients with Femoral Intertrochanteric Fracture

目的探究股骨近端防旋髓内钉内固定术对股骨粗隆间骨折患者功能康复及并发症的影响。方法选取锡山人民医院东亭分院2019年5月—2022年4月期间收治的73例股骨粗隆间骨折患者开展研究,根据不同手术方式分组:接受股骨近端防旋髓内钉内固定术治疗患者为A组(37例)、接受解剖锁定钢板内固定术治疗患者为B组(36例)。对比临床疗效,包括术中情况、术后恢复效果、术后并发症,同时采用髋关节评分量表评价患者术后6个月髋关节功能。结果A组手术时间、切口长度短于对照组,术中出血量少于B组,差异有统计学意义(P<0.05)。A组术后开始负重站立时间(26.21±2.13)d、术后骨折愈合时间(13.29±2.13)d较B组(33.31±3.19)、(15.37±3.11)d减短,术后6个月髋关节Harris评分(91.11±6.09)分较B组(83.29±5.11)分高,差异有统计学意义(P<0.05)。A组术后并发症发生率5.40%较B组的25.00%低,差异有统计学意义(P<0.05)。结论股骨近端防旋髓内钉内固定术的临床疗效较好,利于加速患者术后康复进程,改善髋关节功能,减少术后并发症。

Objective To explore the effect of proximal femoral nail anti-rotation on functional recovery and complications in patients with intertrochanteric fractures.Methods A total of 73 patients with femoral intertrochanteric fractures admitted to Xishan People's Hospital Dongting Branch from May 2019 to April 2022 were selected for research.They were grouped according to different surgical methods:patients who received proximal femoral nail anti-rotation internal fixation were assigned to group A(37 cases),and patients who received anatomical locking plate internal fixation were assigned to group B(36 cases).The clinical curative effects were compared,including intraoperative conditions,postoperative recovery effects,and postoperative complications.At the same time,the Harris Hip score was used to evaluate the hip joint function of the patients 6 months after operation.Results The surgical time and incision length in Group A were shorter than those in the control group,and the intraoperative bleeding was less than that in Group B,the difference was statistically significant(P<0.05).The starting time of weight-bearing and standing(26.21±2.13)d and postoperative fracture healing time(13.29±2.13)d in Group A were shorter than those in Group B(33.31±3.19)d and(15.37±3.11)d after surgery.The Harris score of the hip joint(91.11±6.09)points at 6 months after surgery was higher than that in Group B(83.29±5.11)points,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in Group A 5.40% was lower than that in Group B of 25.00%,and the difference was statistically significant(P<0.05).Conclusion The clinical curative effect of proximal femoral nail anti-rotation fixation is good,which is beneficial to accelerate the postoperative rehabilitation process,improve hip joint function,and reduce postoperative complications.
