彩色多普勒超声诊断下肢深静脉血栓形成后综合征的应用价值分析

Application Value Analysis of Color Doppler Ultrasonography in Diagnosis of Deep Venous Post-thrombotic Syndrome of Lower Extremities

目的分析彩色多普勒超声(clor Doppler ultrasonography,CDUS)诊断下肢深静脉血栓形成后综合征(post-thrombotic syndrome,PTS)的效能。方法选取2020年5月—2022年5月聊城市第四人民医院诊治的84例疑似PTS患者,对所有患者施以CDUS诊断,以数字减影血管造影(digital subtraction angiorgraphy,DSA)为检查的“金标准”,对比CDUS鉴别诊断PTS的效能。结果84例疑似PTS患者经DSA检查证实的有74例,而GDUS共检查出68例;以DSA为“金标准”,GDUS的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为90.54%(67/74)、90.00%(9/10)、90.48%(76/84)、98.53%(67/68)、56.25%(9/16);GDUS检查结果与DSA检查结果的一致性较为理想(Kappa=0.639,P<0.001)。结论GDUS在PTS的鉴别诊断中效果较为显著,可有效鉴别诊断出PTS,与DSA检查结果具有较高的一致性,可作为PTS的一种辅助检查措施,临床应用价值较高。

Objective To analyze the diagnostic efficacy of color Doppler ultrasonography(CDUS)in the diagnosis of deep venous postthrombotic syndrome(PTS)of lower extremities.Methods 84 patients with suspected PTS treated in Liaocheng Fourth People's Hospital from May 2020 to May 2022 were selected,and all patients were diagnosed with CDUS.Digital subtraction angiorgraphy(DSA)was used as the gold standard to compare the efficacy of CDUS in the differential diagnosis of deep venous PTS of lower extremities.Results Of the 84 suspected PTS patients,74 were confirmed by DSA,while 68 were identified by GDUS.With DSA as the"gold standard",the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of GDUS were 90.54%(67/74),90.00%(9/10),90.48%(76/84),98.53%(67/68)and 56.25%(9/16),respectively;the consistency of GDUS test result and DSA test result was ideal(Kappa=0.639,P<0.001).Conclusion GDUS has a significant effect in the differential diagnosis of PTS,which can effectively identify PTS and has a high consistency with the results of DSA.It can be used as an auxiliary examination measure of PTS,which has high clinical application value.
