全程营养支持对鼻咽癌急性放疗反应的影响

Effect of total nutritional support on response to acute radiotherapy for nasopharyngeal carcinoma

目的分析全程营养支持对鼻咽癌急性放疗患者的影响。方法以中山大学肿瘤防治中心放疗科2020年1月至2022年12月收治的鼻咽癌放疗患者92例为研究对象, 采用随机数字表法将其分为对照组与观察组, 每组46例。对照组采用常规营养干预, 观察组采用全程营养支持。于患者放疗1 w与放疗4 w时, 比较两组患者的血清总蛋白、白蛋白及血红蛋白水平, 并使用急性放射反应评分标准及癌症患者生活质量量表对两组患者的毒副反应及生活质量进行评估和比较。结果放疗1 w时, 两组患者相关营养指标含量、癌症生活质量量表评分比较差异均无统计学意义(均P>0.05);放疗4 w时, 观察组相关营养指标高于对照组、放疗症状毒副反应评分低于对照组, 同时观察组癌症生活质量量表中的正性条目评分高于对照组, 负性条目评分低于对照组, 两组比较差异均有统计学意义(均P<0.05)。结论全程营养支持的应用, 能够明显改善鼻咽癌放疗患者的机体营养状况, 缓解放疗症状毒副反应, 同时大幅度提升患者的生活质量。

Objective To analyze the effect of full nutritional support in patients with nasopharyngeal carcinoma(NPC)radiotherapy.Methods A toal of 92 patients with NPC radiotherapy admitted to this hospital from January 2020 to December 2022,they were divided into control group and observation group by random number table.The control group adopted conventional nutritional intervention and the observation group adopted full nutritional support,with 46 cases in each group.At 1 week of radiotherapy and 4 weeks of radiotherapy,the serum total protein,albumin and hemoglobin levels of the two groups were compared,and the scores of the two patients were evaluated using the toxicity and quality of life of the two groups.Results At 1 week of radiotherapy,there was no difference in the relevant nutritional index content and cancer quality of life scale between the two groups(P>0.05);at 4 weeks of radiotherapy,the observation group was higher than the control group,was lower than the control group,the positive item score in the cancer quality of life questionnaire-core(QLQ-C30)was higher than the control group,and the negative item score was lower than that of the control group(P<0.05).Conclusions The application of whole-process nutritional support can significantly improve the nutritional status of NPC patients with radiotherapy,significantly alleviate the symptoms and side effects of radiotherapy,and greatly improve the quality of life.
