反重力跑台减重设备对髋关节置换术后患者运动功能和慢性疼痛的效果研究

摘要 目的:探讨反重力跑台减重设备对髋关节置换术后患者运动功能和慢性疼痛的效果。方法:选取医院收治的86例行髋关节置换术的患者,按照随机数表法将其分为观察组和对照组,每组43例。对照组患者术后实施常规功能康复训练,观察组在对照组康复训练基础上增加反重力跑台训练,对比两组患者术后运动功能的巴塞尔(Barthel)指数、髋关节功能(Harris)评分、疼痛情况以及并发症发生率。结果:两组患者治疗后1个月和3个月时Barthel指数、Harris评分明显升高,治疗后3个月明显高于治疗后1个月和治疗前,且观察组高于对照组,差异有统计学意义(t_(Barthel指数)=2.970,t=3.234;t_(Harris评分)=3.433,t=2.057;P<0.05);两组患者治疗前视觉模拟量表(VAS)评分对比差异无统计学意义,治疗后1个月和3个月时两组患者VAS评分明显降低,治疗后3个月明显低于治疗后1个月和治疗前,且观察组低于对照组,差异有统计学意义(t=3.084,t=2.327;P<0.05;观察组患者慢性疼痛发生率明显低于对照组,差异有统计学意义(x^(2)=6.541,P<0.05);观察组患者并发症发生率明显低于对照组,差异有统计学意义(x^(2)=3.980,P<0.05)。结论:髋关节置换术后患者在常规功能康复训练基础上增加反重力跑台训练能够进一步提升患者运动功能,减轻疼痛感和慢性疼痛发生率,且安全性较高。 Objective:To investigate the effect of anti-gravity treadmill on motor function and chronic pain of patients after hip arthroplasty.Methods:A total of 86 patients after hip arthroplasty in hospital were selected as the research object.All patients were randomly divided into observation group and control group,with 43 cases in each group.The patients in the control group received routinely functional rehabilitation training,and the patients in observation group added the training of anti-gravity treadmill on the basis of the control group.The motor functions,pains and incidences of complication of the two groups were compared.Results:Before treatment,there were no significant differences in Barthel index rating scale(Barthel Index)and Harris score of hip function between the two groups(P>0.05).Barthel Index and Harris score of the two groups increased significantly at the 1st and the 3rd months after treatment,and the two indicators at the 3rd months after treatment were significantly higher than those at the 1st month after treatment and those before treatment,and these of the observation group were significantly higher than them of the control group(t_(Barthel Index)=2.970,t=3.234,t_(Harris score)=3.433,t=2.057,P<0.05),respectively.There was no significant difference in the scores of visual analogue scale(VAS)between the two groups before treatment.The VAS scores of the two groups decreased significantly at the 1st month and the 3rd month after treatment,and the VAS score at the 3rd month after treatment was significantly lower than that at the 1st month and that before treatment,and the VAS score of the observation group was significantly lower than that of the control group(t=3.084,t=2.327,P<0.05),respectively.The incidence of chronic pain in the observation group was significantly lower than that in the control group(x^(2)=6.541,P<0.05).The incidence of complications of the observation group was significantly lower than that of the control group(x^(2)=3.980,P<0.05).Conclusion:The training of anti-gravity treadmill on the basis of routinely functional rehabilitation training for patients after hip arthroplasty can further improve their motor function,and reduce their pain and decrease the incidence of chronic pain,which has higher safety.
