摘要
目的使用微视血管成像技术(microvision blood flow imaging,MFI)联合脉冲多普勒(pulse Doppler technique,PW)定量评估正常中晚孕妊娠胎盘绒毛血管的发育情况。方法选取2021年1~7月在山东省立第三医院进行产科检查的136例单胎妊娠健康中晚孕孕妇作为研究对象,应用MFI充分显示孕妇胎盘绒毛血管树后,启动PW对胎盘绒毛血管分支进行测量,并进行相关统计学分析。结果中孕及晚孕不同位置胎盘绒毛血管具有不同血流参考值;孕周与不同位置胎盘绒毛PSV及S/D相关性较高,其中初级绒毛PSV、次级绒毛PSV、三级绒毛PSV与孕周呈显著正相关(r>0,P<0.01),初级绒毛RI、初级绒毛S/D、次级绒毛S/D与孕周呈显著负相关(r<0,P<0.01),三级绒毛S/D与孕周呈负相关(r<0,P<0.05),初级绒毛PI、次级绒毛PI、三级绒毛PI、次级绒毛RI、三级绒毛RI与孕周无明显相关性(P>0.05)。结论MFI联合PW可测得不同孕周的初级、次级、三级绒毛干血流情况,并且上述数值与孕周相关性较高,对于评估中晚孕胎盘绒毛血管发育情况具有一定的潜力及价值。
Objective To quantitatively evaluate the development of placental villus blood vessels in normal second and third trimester pregnancy by using microvision blood flow imaging(MFI)combined with pulse Doppler technique(PW).Methods A total of 136 healthy second and third trimester pregnancy women with singleton pregnancy who underwent obstetric examination in Shandong Provincial Third Hospital from January to July 2021 were selected as the research subjects.After MFI was used to fully display the placental villus vascular tree of pregnant women,PW was used to analyze the placental villus vascular branches.Measurements were performed and relevant statistical analyses were performed.Results The placental villi vessels at different positions in the second and third trimesters had different blood flow reference values.There was a high correlation between the gestational week and the placental villus PSV and S/D at different positions,among which the primary villus PSV,secondary villus PSV and tertiary villus PSV were significantly positively correlated with the gestational week(r>0,P<0.01),the primary villus RI,primary villus S/D and secondary villus S/D were significantly negatively correlated with the gestational week(r<0,P<0.01),the tertiary villus S/D was negatively correlated with the gestational week(r<0,P<0.05).There was no significant correlation between primary villus PI,secondary villus PI,tertiary villus PI,secondary villus RI,tertiary villus RI and gestational age(P>0.05).Conclusion MFI combined with PW can measure the primary,secondary and tertiary villous stem blood flow at different gestational weeks,and the above values are highly correlated with gestational weeks,which has certain potential and value for evaluating the development of placental villous blood vessels in middle and late pregnancy.
