摘要
目的:观察八脉交会穴之内关、公孙穴配伍使用对功能性消化不良(functional dyspepsia,FD)模型大鼠下丘脑单胺类神经递质去甲肾上腺素(NE)、多巴胺(DA)、5羟色胺(5-HT)的影响,探讨其治疗FD可能的作用机制。方法:将45只SD大鼠随机分为空白组10只和模型储备组35只,模型储备组按复合病因造模法连续造模3周,造模成功后将大鼠随机分为模型组,针刺组、西药组(各10只),电针组行电针内关和公孙穴治疗,西药组予以灌服氟哌噻吨美利曲辛、莫沙必利溶液治疗,1次/d,连续21 d后。分别于造模前后行旷场实验检测大鼠的抑郁样行为,治疗结束后行糖水消耗实验和胃排空实验;采用HPLC法检测下丘脑NE、DA、5-HT含量。结果:与造模前相比,造模后各组大鼠均表现为毛发枯乱发黄,活动减少,扎堆甚至倦卧,大便时干时稀,食欲减退,体重增长变慢;旷场实验示:大鼠站立、穿格、修饰次数明显减少,综上提示FD模型造模成功。与空白组相比,模型组大鼠的糖水消耗率降低明显,胃残留率明显增加,差异有统计学意义(P<0.01);与模型组比,针刺组、西药组大鼠的糖水消耗率显著升高,胃残留率明显减少,差异有统计学意义(P<0.01),但是针刺组与西药组之间差异无统计学意义(P>0.05)。与空白组比较,模型组鼠下丘脑NE、DA、5-HT含量显著下降,差异有统计学意义(P<0.01);与模型组相比,针刺组、西药组大鼠下丘脑NE、DA、5-HT含量显著的上升,差异有统计学意义(P<0.01),两治疗组差异无统计学意义(P>0.05)。结论:八脉交会穴之内关、公孙穴配伍使用能有效改善FD大鼠的胃肠动力及其伴随的抑郁样行为,其作用机制可能与增加下丘脑单胺类神经递质的NE、DA、5-HT含量,从而纠正HPA轴功能有关。
Objective To observe the effects of the compatibility of Neiguan and Gongsun points at the intersection of the eight meridians on the hypothalamic monoamine neurotransmitters norepinephrine(NE),dopamine(DA),and serotonin(5-HT)in rats with functional dyspepsia(FD),and to explore the possible mechanism of their effects on FD.Methods A total of 45 SD rats were randomly divided into a blank group(n=10)and a model retention group(n=35).The model reserve group was established for three weeks according to the method of compound etiology modeling.They were randomly divided into model group,Acupuncture Group and Western medicine group with 10 rats in each group,after successful modeling in the Model Reserve Group.The electroacupuncture group was treated with electroacupuncture at Neiguan and Gongsun points,and the western medicine group was treated with oral administration of Deanxit and Mosapride solution once a day for 21 days.The depression like behavior of rats was detected by open field test before and after the model establishment,and the sugar water consumption test and gastric emptying test were conducted after the treatment;Detecting the contents of NE,DA and 5-HT in hypothalamus by HPLC.Results Compared with the rats before modeling,the rats in each group after modeling showed that their hair was withered and yellow,their activities were reduced,they were clumped up or even tired to lie down,their stools were dry and thin,their appetite was decreased,and their weight growth was slower.The open field experiment showed that the number of times of rats standing,wearing and modifying was significantly reduced,which indicated that the FD model was successfully established.Compared with the blank group,the consumption rate of sugar water in the model group was significantly reduced,and the rate of stomach remain increased significantly(P<0.01).Compared with the model group,the consumption rate of sugar water in the acupuncture group and the western medicine group increased significantly,and the gastric residual rate decreased remarkably(P<0.01).However,the two treatment groups was no clear difference(P>0.05).Compared with the blank group,the decrease is very obvious in the contents of NE,DA and 5-HT in hypothalamus of model group(P<0.01).In contrast to the model group,the contents of NE,DA and 5-HT in hypothalamus of rats in acupuncture group and western medicine group increased remarkably(P<0.01),however,the two treatment groups was no clear difference(P>0.05).Conclusion The combination of Neiguan and Gongsun acupoints of the Bamai Jiaohui acupoints can effectively improve the gastrointestinal motility of FD rats and their concomitant depressive behavior,and the mechanism of action is perhaps have a bearing onthe content of NE,DA,5-HT monoamine neurotransmitters in the hypothalamus,thus correcting the function of HPA axis.
