地塞米松鼓室内注射治疗内耳眩晕的临床疗效研究

Clinical effect of intratympanic injection of dexamethasone on vertigo of inner ear

目的探讨地塞米松鼓室内注射治疗内耳眩晕的临床疗效。方法选取2021年6月-2022年10月收治的125例内耳眩晕患者为研究对象,以统计学软件的随机分组功能将患者分成对照组(n=62)和观察组(n=63)两组。其中对照组以碳酸氢钠治疗,观察组在此基础上联合地塞米松治疗。对比两组眩晕治疗效果、听力干预效果、耳鸣干预效果、不良反应。结果观察组眩晕治疗总有效率90.48%(57/63),对照组治疗总有效率72.58%(45/62),观察组高于对照组(P<0.05)。观察组听力改善率69.84%(44/63),对照组听力改善率45.16%(28/62),观察组高于对照组(P<0.05)。治疗前两组听阈水平对比差异不显著(P>0.05),治疗后观察组听阈水平低于对照组(P<0.05)。观察组耳鸣干预缓解率80.95%(51/63),对照组耳鸣干预缓解率40.32%(25/62),观察组高于对照组(P<0.05)。两组不良反应发生率均较低,且对比差异不显著(P>0.05)。结论常规治疗方案上增加地塞米松鼓室内注射治疗内耳眩晕具有较为理想的治疗效果,可改善患者眩晕、耳鸣和听力下降问题,并且局部用药具有较高的安全性。

Objective To investigate the clinical effect of intratympanic injection of dexamethasone in the treatment of vertigo of the inner ear.Methods 125 patients with vertigo of the inner ear admitted from June 2021 to October 2022 were selected as the study subjects.The patients were divided into two groups by the random grouping function of statistical software,the control group(n=62)and the observation group(n=63).The control group was treated with sodium bicarbonate,and the observation group was treated with dexamethasone on this basis.The effects of vertigo treatment,hearing intervention,tinnitus intervention and adverse reactions were compared between the two groups.Results The total effective rate of vertigo treatment in the observation group was 90.48%(57/63),and that in the control group was 72.58%(45/62),which was higher than that in the control group(P<0.05).The improvement rate of hearing in the observation group was 69.84%(44/63),while that in the control group was 45.16%(28/62).The improvement rate of hearing in the observation group was higher than that in the control group(P<0.05).There was no significant difference in the level of hearing threshold between the two groups before treatment(P>0.05);after treatment,the hearing threshold level in the observation group was lower than that in the control group(P<0.05).The intervention remission rate of tinnitus in the observation group was 80.95%(51/63),and the intervention remission rate of tinnitus in the control group was 40.32%(25/62),which was higher in the observation group than in the control group(P<0.05).The incidence of adverse reactions in both groups was low,and the difference was not significant(P>0.05).Conclusion Intratympanic injection of dexamethasone in the treatment of vertigo of the inner ear in the conventional treatment scheme has an ideal therapeutic effect,which can improve the problems of vertigo,tinnitus and hearing loss of patients,and local medication has high safety.
