二陈汤合三子养亲汤加减联合孟鲁司特钠对痰湿蕴肺型喘症患者的肺功能及外周血Th17/Treg水平的影响

Effects of Erchen Decoction and Sanzi Yangqin Decoction combined with Montelukast Sodium on lung function and Th17/Treg level in peripheral blood of patients with phlegm-dampness pulmonary asthmatic disease

目的探讨二陈汤合三子养亲汤加减联合孟鲁司特钠对痰湿蕴肺型喘症患者的肺功能及外周血Th17/Treg水平的影响。方法选取2021年8月-2022年8月在上犹县中医院确诊的60例痰湿蕴肺型喘症患者,采用简单随机分组法将患者分为联合组和对照组,每组各30例。对照组患者服用孟鲁司特钠,联合组在对照组的基础上加用二陈汤合三子养亲汤加减治疗,均持续治疗6周。比较两组治疗前和治疗6周后肺功能指标、外周血Th17/Treg水平、炎症因子水平和主要中医证候积分,并记录两组患者不良反应。结果治疗后,联合组患者的FEV1、FVC和FEV1/FVC均高于对照组患者,组间差异有统计学意义(P<0.05)。治疗后,联合组患者外周血中Th17百分比低于对照组,Treg百分比高于对照组,联合组的Th17/Treg水平低于对照组,组间差异有统计学意义(P<0.05)。治疗后,联合组患者TNF-α、WBC和IL-6水平均低于对照组,组间差异有统计学意义(P<0.05)。联合组患者的不良反应率为13.33%,对照组为16.67%,两组比较,差异无统计学意义(P>0.05)。结论二陈汤合三子养亲汤加减联合孟鲁司特钠治疗痰湿蕴肺型喘症患者可以改善肺部功能和炎症状态,调节患者的免疫功能,且治疗方法无明显不良反应,具有一定的安全性。

Objective To investigate the effects of Erchen Tang and Sanzi Yangqin Tang combined with Montelukast sodium on lung function and Th17/Treg level in peripheral blood of patients with phlegm-dampness pulmonary asthmatic syndrome.Methods A total of 60 patients with phlegm-damp-pulmonary asthmatic disease diagnosed in Shangshu County Hospital of Traditional Chinese Medicine from August 2021 to August 2022 were selected and randomly divided into a combination group and a control group,with 30 cases in each group.The control group received montelukast sodium,and the combined group was additionally treated with Erchen Decoction and Sanzi Yangqin Decoction for 6 weeks.Pulmonary function indexes,peripheral blood Th17/Treg levels,inflammatory factors and major TCM symptom scores were compared between the two groups before and 6 weeks after treatment,and adverse reactions were recorded between the two groups.Results After treatment,FEV1,FVC and FEV1/FVC in combination group were higher than those in control group,and the difference between groups was statistically significant(P<0.05).After treatment,the Th17 percentage in peripheral blood of patients in combination group was lower than that in control group,the Treg percentage was higher than that in control group,and the Th17/Treg level in combination group was lower than that in control group,the difference between groups was statistically significant(P<0.05).After treatment,the levels of TNF-α,WBC and IL-6 in combination group were lower than those in control group,and the difference between groups was statistically significant(P<0.05).The adverse reaction rate was 13.33%in the combination group and 16.67%in the control group,and there was no statistical significance between the two groups(P>0.05).Conclusion Erchen Decoction combined with Sanzi Yangqin Decoction combined with Montelukast sodium in the treatment of phlegm-dampness pulmonary asthmatic patients can improve lung function and inflammatory state,regulate the immune function of patients,and the treatment has no obvious adverse reactions,so it has certain safety.
