摘要
目的观察577 nm阈下微脉冲激光(SML)治疗干性年龄相关性黄斑变性玻璃膜疣的临床疗效。方法回顾性分析28例以典型玻璃膜疣为特征的干性年龄相关性黄斑变性患者。将接受577 nm SML治疗的14例设为SML组,单纯行随访观察的14例设为对照组。比较分析两组临床资料。结果与就诊前比较,对照组随访6个月后患眼玻璃膜疣面积增加,P<0.01,而SML组治疗6个月后患眼玻璃膜疣面积减少,P<0.05。与就诊前比较,对照组随访6个月后患眼玻璃膜疣体积增加,P<0.01,而SML组治疗6个月后患眼玻璃膜疣体积减少,P<0.01。治疗6个月后,SML组患眼玻璃膜疣体积小于对照组,P<0.01。SML组治疗6个月后患眼黄斑中心视网膜厚度(CMT)减少,P<0.01。就诊6个月后,SML组患眼CMT小于对照组,P<0.05。治疗6个月后,SML组患眼振幅小于对照组,P<0.01。与就诊前比较,对照组随访6个月后患眼潜伏期增加,P<0.05。SML组、对照组均未发现脉络膜新生血管。治疗6个月后,SML组黄斑峰较治疗前升高,局部振幅改善。结论577 nm SML可有效减少干性年龄相关性黄斑变性玻璃膜疣的面积、体积。
Objective To observe the effect of 577 nm subliminal micropulse laser(SML)in the treatment of dry age-related macular degeneration(AMD)drusen.Methods Totally 28 cases(34 eyes)of AMD patients were retrospectively analyzed.According to different intervention methods,14 cases of 577 nm micropulse laser photocoagulation were set as the SML group,14 cases of follow-up observation alone were set as the control group.The clinical data of the two groups were compared and analyzed.Results Compared with before treatment,the area of glass wart increased in the control group after 6 months of treatment,P<0.01,while the area of glass wart decreased in the SML group after 6 months of treatment,P<0.05.Compared with before treatment,the volume of glass film warts increased in the control group after 6 months of treatment,while the volume of glass film warts decreased in the SML group after 6 months of treatment,P<0.01.After 6 months of treatment,the volume of glass membrane warts in SML group was smaller than that in control group,P<0.01.In SML group,central macular notch thickness,(CMT)decreased after 6 months of treatment,P<0.01.After 6 months of treatment,CMT in SML group was lower than that in control group,P<0.05.After 6 months of treatment,the amplitude of affected eye in SML group was lower than that in control group,P<0.01.Compared with before treatment,the incubation period of affected eye in control group increased after 6 months of treatment,P<0.05.No choroidal neovascularization was found in SML group and control group.After 6 months of treatment,the macular peak in SML group was higher than before treatment,and the local amplitude was improved.Conclusion 577 nm micropulse laser can effectively reduce the area and volume of drusen in dry age-related macular degeneration.
