中国西部地区涉外医疗现状调查及规范化管理模式探讨

Investigation on the current situation of foreign-related medical treatment in western China and discussion on standardized management mode

目的分析中国西部地区涉外患者诊疗现状,探讨中国西部地区涉外医疗规范化管理模式。方法选取2017—2022年度在西安交通大学第一附属医院就诊的124例涉外患者作为研究对象,根据入住病房不同将其分为研究组(n=63,入住涉外病房,标准化住院涉外患者管理模式)和对照组(n=61,入住普通病房,常规住院管理模式)。分析124例涉外患者的就诊现状及存在问题;比较两组患者入院时及随访12周后的生活质量。结果“语言交流障碍”和“隐私保护不够”对涉外患者的就医体验影响较大,有待进一步加强。75.8%患者认为和中国人交流“存在困难”,14.5%患者认为“非常困难”,2.4%患者认为“不能交流”,仅有7.3%患者认为“没有困难”。大部分患者认为在中国就诊非常便捷,预约检查时间短,完成诊疗周期短,仅有8.9%患者认为“不便捷”。随访12周后,研究组的生理、心理、独立性评分及总分高于对照组,差异具有统计学意义(P<0.05)。结论我国西部地区涉外诊疗现状不容乐观,探索和建立规范化涉外医疗管理模式,有助于提高涉外患者就医体验和满意度,为涉外医疗的发展提供建设性意见。

Objective To analyze the current situation of diagnosis and treatment of foreign-related patients in western China,and to explore the standardized management mode of foreign-related medical treatment in western China.Methods A total of 124 foreign-related patients who were admitted in the First Affiliated Hospital of Xi'an Jiaotong University from 2017 to 2022 were selected as the research objects.According to the different admission wards,the patients were divided into study group(n=63,admitted in the foreign-related ward,standardized foreign-related patients management mode)and control group(n=61,admitted in the general ward,routine hospitalization management mode).The current situation of medical treatment and existing problems of 124 foreign-related patients were analyzed;the quality of life at admission and 12 weeks after follow-up was compared between the two groups.Results"Language communication barriers"and"insufficient privacy protection"had impact on the medical experience of foreign-related patients,and need to be further strengthened.75.8%of patients thought it was"difficult"to communicate with Chinese people,14.5%of patients thought it was"very difficult",2.4%of patients thought it was"impossible to communicate",and only 7.3%of patients thought it was"not difficult".Most of the patients thought that it was very convenient to visit a doctor in China,with short appointment time and short completion cycle,while only 8.9%of patients thought it was"inconvenient".After 12 weeks of follow-up,the physiology,psychology and independence scores and total score of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The current situation of foreign-related diagnosis and treatment in western China is not optimistic.Exploring and establishing a standardized foreign-related medical management mode will help to improve the medical experience and satisfaction of foreign-related patients,and provide constructive suggestions for the development of foreign-related medical treatment.
