围术期一体化保温护理管理在老年髋关节置换术中的应用效果

Application effect of perioperative integrated thermal insulation nursing management in elderly patients undergoing hip replacement

目的探讨围术期一体化保温护理管理在老年髋关节置换术中的应用效果。方法选取2020年4月至2021年3月我院拟实施髋关节置换术的68例老年患者为研究对象,按随机数字表法将其分为对照组与研究组,各34例。对照组采取常规手术室护理,研究组采取围术期一体化保温护理管理。比较两组不同时间点的鼓膜温度、术前术后的凝血功能指标、术中失血量、总尿量及升压药使用量、不良事件发生情况以及护理满意度评分。结果手术开始时(T_(3))、术中30 min(T_(4))、术中1 h(T_(5))、手术结束即刻(T_(6))时,研究组的鼓膜温度均高于对照组(P<0.05)。术后,研究组的纤维蛋白原(FIB)水平高于对照组,凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)均短于对照组(P<0.05)。研究组的术中失血量、升压药使用量少于对照组,总尿量多于对照组(P<0.05)。研究组的不良事件总发生率低于对照组(P<0.05)。研究组的护理满意度评分显著高于对照组(P<0.05)。结论采取围术期一体化保温护理管理对老年髋关节置换术患者实施干预,可有效维持患者体温恒定,改善凝血功能,减少不良事件的发生,进而提高患者满意度。

Objective To explore the application effect of perioperative integrated thermal insulation nursing in elderly patients undergoing hip replacement.Methods A total of 68 elderly patients who underwent hip replacement in our hospital from April 2020 to March 2021 were selected as the research objects,and the patients were divided into control group and study group according to the random number table method,with 34 cases in each group.The control group adopted routine operating room nursing,and the study group adopted perioperative integrated thermal insulation nursing management.The tympanic membrane temperature at different time points,preoperative and postoperative coagulation function indexes,intraoperative blood loss,total urine volume and booster usage,adverse events and nursing satisfaction scores were compared between the two groups.Results At the beginning of operation(T_(3)),30 min during operation(T_(4)),1 h during operation(T_(5))and immediately after operation(T_(6)),the tympanic membrane temperature of the study group was higher than that of the control group(P<0.05).After operation,the level of fibrinogen(FIB)in the study group was higher than that in the control group,and thrombin time(TT),activated partial thromboplastin time(APTT)and prothrombin time(PT)were shorter than those in the control group(P<0.05).The intraoperative blood loss and booster usage in the study group were less than those in the control group,and the total urine volume was more than that in the control group(P<0.05).The total incidence of adverse events in the study group was lower than that in the control group(P<0.05).The nursing satisfaction score of the study group was significantly higher than that of the control group(P<0.05).Conclusion Implementing perioperative integrated thermal insulation nursing management for elderly patients with hip replacement can effectively maintain the constant body temperature,improve coagulation function,reduce the occurrence of adverse events,and improve patients'satisfaction.
