基于五运六气学说探讨肛瘘患者的先天禀赋

An analysis of the characteristics of natural endowment of patients with anal fistula based on the five movements and six Qi theory

目的:肛瘘在临床以局部反复流脓、疼痛、瘙痒为主要症状,中医中药对于肛瘘的防治有重要意义。文章探讨肛瘘患者在出生日期的五运六气禀赋特征,以求发现肛瘘患者的先天运气相关规律,为肛瘘防范治疗提供新方向新思路。方法:通过参考相关估计记载和文献研究,对南京市中医院4252例肛瘘患者的出生日期进行干支纪运转化,推算出相应的五运六气,并对其进行描述性统计以及卡方检验。结果:肛瘘患者的出生日期在天干、主气、司天在泉方面的分布差异具有统计学意义(P<0.05)。易患肛瘘的出生日期年干主要为壬年、已年即木运太过、土运不及之年,主气为五之气阳明燥金,司天在泉为太阳寒水司天,太阴湿土在泉。不易患肛瘘的出生日期年干为乙年及金运不及之年,主气为二之气少阴君火,司天在泉为太阴湿土司天、太阳寒水在泉。结论:不同出生年患者的运气体质禀赋与后天罹患肛瘘有相关性,分析得出易患肛瘘的体质特点,有助于针对患者不同五运六气禀赋特点给予个性化防治措施,对肛瘘的治疗及预防复发有一定参考意义。

Objective:Fistula is mainly characterized by local repeated pus,pain and itching in clinical practice,and TCM medicine is of great significance for the prevention and treatment of fistula.To investigate characteristics of natural endowment of patients with anal fistula on the date of birth based on the five movements and six Qi(五运六气)theory.Methods:With reference to relevant ancient books and literature studies,the birth dates of 4252 patients with anal fistula in Nanjing Hospital of traditional Chinese Medicine were converted into heavenly stems and earthly branches,and the five movements and six Qi were calculated,and descriptive statistics and chi-square test were carried out.Results:There was significant difference in the birth date of patients with anal fistula in heavenly stems,host climatic Qi(气)and celestial manager Qi(P<0.05).Among the birth dates of those who are prone to anal fistula,the main years are Ren(壬)and Ji(己)years,that is,the years when wood motion is too much and earth reign is less than.Host climatic Qi is correspond to the Yangming Zaojin(阳明燥金).Celestial manager Qi for Taiyang Hanshui(太阳寒水)in Sitian(司天),Taiyin Shitu(太阴湿土)in Zaiquan(在泉).In the date of birth of people who are not easy to suffer from anal fistula,the main year is the Yi(乙)year,that is,the year when gold movement is less than,host climatic Qi is Shaoyin Junhuo(少阴君火),celestial manager Qi for Taiyin Shitu in Sitian,Taiyang Hanshui in Zaiquan.Conclusion:There is a certain correlation between the heavenly stems,five elements'motion and six kinds of natural factors,natural endowment at birth and the occurrence of anal fistula.The five movements and six Qi theory can be used for the prevention and treatment of anal fistula patients with different natural endowments.
