从“胆气春升”论治成人夜惊症验案一则

One empirical case of treating adult night terrors from the perspective of the vitality of the gallbladder increases with the arrival of spring theory

夜惊症是指突然从深睡中醒来,并伴有因强烈恐惧产生的尖叫、异常行为和交感神经功能亢进的症状。近年来随着经济发展、竞争激烈和压力的上升,睡眠障碍的发生率在不断升高,夜惊症的治愈率不高。长期而持续的夜惊可能导致焦虑、抑郁,严重者可引起神经衰弱、睡眠障碍等一系列问题。夜惊症是儿童常见疾病,成人较为罕见。文章介绍一例成人夜惊症的治疗案例,患者病程2年。李杲提出了胆气宜升宜足,后世之人将其归纳为“胆气春升”。该理论体现了胆的“春生”与“升清”之性。夜惊症的发生可归纳为胆失春升,情志内伤,则决断迟疑,情志失调,抵抗外邪无力,易受邪犯。笔者以“胆气春升”为指导原则,以“夜惊症”(胆气不足型)论治,取得了显著疗效,遂详细记录为本案,以期为临床诊疗成人夜惊症提供新思路。

Night terrors refer to sudden awakening from deep sleep,accompanied by screams,abnormal behaviors and symptoms of sympathetic hyperfunction caused by strong fear.In recent years,with the economic development,fierce competition and rising pressure,the incidence of sleep disorders is increasing,and the cure rate of night terrors is not high.Long-term and continuous night terrors may lead to daytime anxiety and depression,and a series of problems such as neurasthenia and sleep disorders may occur in severe cases.Night terrors is a common disease in childhood,which is rare in adults.This paper introduces one empirical case of adult night terrors.The course of the disease lasted for about 2 years.LI Gao(李杲)proposed that bile should rise and be sufficient,and later generations summarized it as the vitality of the gallbladder increases with the arrival of spring.This theory embodies the develop in spring and Shengqing(升清)nature of gall.The occurrence of night terror disease can be summarized as the vitality of the gallbladder increases with the arrival of spring,emotional internal injury,decision hesitation,emotional disorder,inability to resist external evil,and vulnerability to evil offenders.Based on the guiding principle of the vitality of the gallbladder increases with the arrival of spring theory,the author treated night terrors of gallbladder Qi(气)insufficiency type and achieved remarkable results.Therefore,this paper will share this case in order to provide a clinical reference.
