摘要
目的:探讨气管支架植入术后继发肺炎的危险因素及病原学特征。方法:选取2015年4月-2022年11月于宜昌市中心人民医院确诊为气管、支气管狭窄并行支架植入的421例患者,根据是否术后继发肺炎分组,其中术后继发肺炎的为病例组(n=96),术后未继发肺炎的为对照组(n=325),收集两组患者相关资料进行Logistics回归分析。结果:多因素分析显示,恶性气道狭窄及患有慢性阻塞性肺疾病(COPD)是气管支架植入术后继发肺炎的独立危险因素。96例肺炎患者中有71例(73.96%)共培养出83株致病菌,其中以革兰氏阴性杆菌中的铜绿假单胞菌为主。结论:恶性气道狭窄及COPD可增加气管支架植入术后继发肺炎的发生风险,其致病菌以铜绿假单胞菌为主。
Objective:To investigate the risk factors and etiological characteristics of secondary pneumonia after tracheal stent implantation.Methods:A total of 421 patients underwent stent implantation who were diagnosed with tracheal and bronchial stenosis in Yichang Central People's Hospital from April 2015 to November 2022 were selected.Patients were divided into two groups according to whether they had secondary pneumonia after tracheal stent implantation.There were 96 patients with postoperative secondary pneumonia in the case group,and 325 patients without postoperative secondary pneumonia in the control group.Relevant data were collected for Logistics regression analysis.Results:Multivariate analysis suggested that malignant airway stenosis and chronic obstructive pulmonary disease(COPD)were independent risk factors for secondary pneumonia after tracheal stent implantation.A total of 83 pathogenic strains were cultured from 71(73.96%)of 96 patients with pneumonia,among which Gram-negative Pseudomonas aeruginosa was the main pathogen.Conclusion:Malignant airway stenosis and COPD increases the risk of secondary pneumonia after tracheal stent implantation,with the main pathogenic bacteria of Gram-negative bacillus Pseudomonas aeruginosa.
