摘要
目的:比较3.0 T磁共振血管造影(MRA)与1.5 T MRA对未破裂颅内动脉瘤(IA)的诊断价值。方法:回顾性分析在宜昌市中心人民医院同时行MRA和数字减影血管造影(DSA)检查的患者影像学资料。以DSA为金标准,比较3.0 T MRA与1.5 T MRA对IA及其瘤颈、子囊的检出率。同时比较两种方法与DSA测量IA最长直径(LD)的一致性和相关性。结果:3.0 T MRA和1.5 T MRA的IA检出率分别为89.92%和85.05%,差异无统计学意义(P>0.05)。3.0 T MRA瘤颈、子囊检出率分别为59.81%、26.17%,显著高于1.5 T MRA的38.46%、15.93%(均P<0.05)。3.0 T MRA、1.5 T MRA与DSA测量瘤体LD呈高度正相关,其相关性可达0.984和0.983;3.0 T MRA、1.5 T MRA与DSA测量瘤体LD差值分别为0.30(95%CI:-0.35,0.94)mm和0.66(95%CI:0.12,1.21)mm。1.5 T MRA的LDMRA/LDDSA值较3.0 T MRA更小,差异有统计学意义(P<0.05)。结论:3.0 T MRA与1.5 T MRA在IA的检出上无差异,但3.0 T MRA对IA瘤颈、子囊的检出率更高、形态学指标测量也更加精确。
Objective:To compare the diagnostic value of 3.0 T magnetic resonance angiography(MRA)and 1.5 T MRA in unruptured intracranial aneurysms(IA).Methods:The imaging data of patients who both underwent MRA and digital subtraction angiography(DSA)in Yichang Central People's Hospital were analyzed retrospectively.Taking DSA as the gold standard,the detection rates of IA,its tumor neck and bursa were compared between 3.0 T MRA and 1.5 T MRA.The consistency and correlation between the two methods and DSA in measuring the longest diameter(LD)of IA were analyzed.Results:The detection rates of IA were 89.92%and 85.05%in 3.0 T MRA and 1.5 T MRA,respectively,with no significant difference between two groups(P>0.05).The detection rates of tumor neck and bursa were 59.81%and 26.17%in 3.0 T MRA,which were significantly higher than 38.46%and 15.93%in 1.5 T MRA(both P<0.05).There was a high positive correlation of measured LD between MRA and DSA,and the correlation can reach to 0.984 in 3.0 T MRA and 0.983 in 1.5 T MRA.The difference in measured tumor LD between MRA and DSA was 0.30(95%CI:-0.35,0.94)mm in 3.0 T MRA and 0.66(95%CI:0.12,1.21)mm in 1.5 T MRA.The value of LDMRA/LDDSA in 1.5 T MRA was smaller than that of 3.0 T MRA(P<0.05).Conclusion:There is no significant difference in the detection of IA between 3.0 T MRA and 1.5 T MRA.But in 3.0 T MRA,the detection rate of IA tumor neck and bursa is higher,and the measurement of morphological indexes is more accurate.
