八聚体结合转录因子4、纺锤体和动粒相关蛋白2在肝癌癌变过程中的表达及其临床意义

Expression of octamer-binding transcription factor 4,spindle and kinetochore associated protein 2 in the carcinogenesis of primary liver carcinoma and its relationship with prognosis

摘要 目的探讨八聚体结合转录因子4(OCT4)、纺锤体和动粒相关蛋白2(SKA2)在原发性肝癌(PLC)组织中的表达,及其与PLC临床病理特征和预后的关系。方法采用免疫组织化学法检测2018年1月至2022年11月大庆龙南医院(齐齐哈尔医学院第五附属医院)和广东医科大学附属医院病理确诊的27例正常肝组织、44例肝硬化组织、87例PLC组织中OCT4和SKA2表达,结合临床资料、采用χ^(2)检验分析两者的表达与PLC患者临床病理特征和预后的关系。结果OCT4在正常肝组织、肝硬化组织与PLC组织中阳性表达率分别为11.11%(3/27)、36.36%(16/44)、67.82%(59/87),两两之间比较差异有统计学意义(χ^(2)=11.811、5.444、26.708,P<0.05)。SKA2在正常肝组织、肝硬化组织与PLC组织中阳性表达率分别为14.82%(4/27)、43.19%(19/44)、73.56%(64/87),两两之间比较差异有统计学意义(χ^(2)=11.618、29.547、6.148,P<0.05)。OCT4表达水平与PLC患者TNM分期(TNMstage)、癌细胞分化程度、血管浸润明显相关(χ^(2)=8.182、7.205、6.321,P<0.05),OCT4表达水平与PLC患者性别、年龄、肿瘤大小、病理类型无相关(χ^(2)=0.001、0.049、0.037、0.057,P>0.05)。SKA2表达水平与PLC患者肿瘤大小、TNM分期、血管浸润明显相关(χ^(2)=7.325、5.539、5.256,P<0.05),SKA2表达水平与PLC患者性别、年龄、癌细胞分化程度、病理类型无相关(χ^(2)=0.044、0.003、0.836、0.028,P>0.05)。结论PLC组织中OCT4和SKA2表达水平升高,OCT4表达水平与PLC患者癌细胞分化程度、TNM分期、血管浸润明显相关,SKA2表达水平与PLC患者血管浸润、TNM分期、肿瘤大小明显相关,两者均为PLC患者预后不良的影响因素。 Objective To investigate the expression of octamer-binding transcription factor 4(OCT4)and spindle and kinetochore associated protein 2(SKA2)in primary liver carcinoma(PLC),and their relationship with clinicopathological features and prognosis of PLC.Methods Immunohistochemistry was used to detect the expression of OCT4 and SKA2 in 27 cases of normal liver tissues,44 cases of liver cirrhosis tissues and 87 cases of PLC tissues confirmed by pathology in Daqing Longnan Hospital(Fifth Af-filiated Hospital of Qiqihar Medical College)and Affiliated Hospital of Guangdong Medical University from January 2018 to November 2022.Combined with clinical data,Chi-square test was used to analyze the re-lationship between the expression of these two proteins and the clinicopathological characteristics and prog-nosis of PLC patients.Results The positive expression rate of OCT4 in normal liver tissues,cirrhosis tis-sues and PLC tissues was 11.11%(3/27),36.36%(16/44)and 67.82%(59/87)respectively,and the differences among them were statistically significant(respectively,χ^(2)=11.811,5.444,26.708,P<0.05).The positive expression rate of SKA2 in normal liver tissues,liver cirrhosis tissues and PLC tissues were 14.82%(4/27),43.19%(19/44)and 73.56%(64/87),and the differences among them were statistically significant(respectively,χ^(2)=11.618,29.547,6.148,P<0.05).The expression level of OCT4 was significantly correlated with tumor node metastasis(TNM)stage,differentiation degree of cancer cells and vascular invasion in PLC patients(respectively,χ^(2)=8.182,7.205,6.321,P<0.05).The expression level of OCT4 was not significantly correlated with gender,age,tumor size and pathological type of PLC patients(respectively,χ^(2)=0.001,0.049,0.037,0.057,P>0.05).The expression level of SKA2 was significantly correlated with tumor size,TNM stage and vascular invasion in PLC patients(respectively,χ^(2)=7.325,5.539,5.256,P<0.05).The expression level of SKA2 was not significantly correlated with gender,age,differentiation degree of cancer cells and pathological type of PLC patients(respectively,χ^(2)=0.044,0.003,0.836,0.028,P>0.05).Conclusion The expresson levels of OCT4 and SKA2 in PLC tissues are increased.The expression level of OCT4 is significantly correlated with the degree of cancer cell differentiation,TNM stage and vascular invasion in PLC patients,and that of SKA2 is significantly correlated with vascular invasion,TNM stage and tumor size in PLC patients.Both of them are influencing factors of poor prognosis in PLC patients.
