PCT、CRP和CPIS在呼吸系统感染中的诊断价值和相关性分析 被引量:1

Diagnostic value and correlation analysis of PCT,CRP and CPIS in respiratory system infection

摘要 目的探究降钙素原(procalcitonin,PCT)和C反应蛋白(c-reactionprotein,CRP)在呼吸系统感染中的表达意义和诊断价值,分析其与肺部感染评分(clinic pulmonary infection score,CPIS)的相关性。方法选取2020年7月—2021年7月河北省某医院的150例呼吸系统感染患者,根据是否存在细菌感染,分为细菌感染组和非细菌感染组(n细菌感染=85,n非细菌感染=65);根据感染严重程度,分为轻度感染组(n上呼吸道=38,n下呼吸道=53)和重度感染组(n上呼吸道=24,n下呼吸道=35);根据CPIS,分为≤6分组(n=95)和>6分组(n=55);将80例健康者作为对照组。应用免疫比浊法检测CRP,化学发光免疫法检测PCT。对比CRP、PCT和CPIS,分析三者在呼吸系统感染中的诊断价值和相关性。结果细菌感染组CRP和PCT比非感染组显著升高,差异有统计学意义(F=-16.523、-17.525,P<0.05),细菌感染组CPIS分值明显高于非细菌感染组(t=-24.509,P<0.05);CPIS>6分组比≤6分组,CRP、PCT、CPIS均显著升高,差异有统计学意义(t=-15.371、-6.013、-27.067,P<0.05);下呼吸道感染组比上呼吸道感染组CPIS分值高(t=-2.776,P<0.05),两者CRP和PCT差异无统计学意义(t=0.109、0.220、-0.283、-0.123,P>0.05);联合CRP和PCT检测对呼吸系统感染诊断价值显著高于单独检测,差异有统计学意义(Z=2.691,P<0.05);呼吸系统感染患者CRP、PCT和CPIS呈显著正相关(r=0.927、0.831,P<0.05)。结论呼吸系统感染患者CRP和PCT显著升高,两者联合检测诊断价值高;病情越重,两者表达越高,与CPIS呈正相关,三者联合检测可作为预测患者病情及严重程度的重要指标,值得在临床推广应用。 Objective To explore the expression significance and diagnostic value of procalcitonin(PCT)and C-reactive protein(CRP)in respiratory infections,and analyze their correlation with pulmonary infection(CPIS)scores.Methods A total of 150 patients with respiratory system infections admitted to a hospital in Hebei Province were selected from July 2020 to July 2021,and they were divided into bacterial infection group(n=85)and non-bacterial infection group(n=65)according to bacterial infection􀆰The infected patients were divided into mild infection group(nupper respiratory tract=38,nlower respiratory tract=53)and severe infection group(nupper respiratory tract=24,nlower respiratory tract=35).According to the CPIS score,they were divided into groups with CPIS score≤6(n=95)and groups with CPIS score>6(n=55),and 80 healthy individuals were selected as the control group.CRP was detected by immunoturbidimetry,and PCT was detected by chemiluminescence immunoassay.The CRP,PCT and CPIS scores of different groups were compared to analyze their diagnostic value and correlation in respiratory sustem infection.Results Compared to the non-bacterial infection group,the CRP and PCT of the bacterial infection group were significantly increased,and the difference was statistically significant(F=-16.523,-17􀆰525,P<0.05).The CPIS score of the bacterial infection group was significantly higher than that of the non-bacterial infection group(t=-24.509,P<0.05).Compared with the patients in the group of CPIS score≤6,the CRP,PCT and CPIS scores of the patients in the group of CPIS score>6 were significantly increased,and the difference was statistically significant(t=-15.371,-6.013,-27.067,P<0.05).The CPIS score of lower respiratory tract infection was significantly higher than those with upper respiratory tract infection(t=-2.776,P<0.05),and there was no significant difference in CRP and PCT between the two groups(t=0.109,0.220,-0.283,-0.123,P>0.05).The value of combined CRP and PCT in the diagnosis and evaluation of respiratory system infection was significantly higher than that of single detection,and the difference was statistically significant(Z=2.691,P<0.05).There was a significant positive correlation between CRP and PCT and CPIS scores in patients with respiratory system infection(r=0.927,0.831,P<0.05).Conclusion The expressions of CRP and PCT in patients with respiratory system infection have significantly increased,and the combined CRP and PCT detection of respiratory system infection is of higher diagnostic value.The more serious the patient′s condition is,the higher the expression of CRP and PCT would be,and it is positively correlated with CPIS score􀆰The combination detection of the three can be used as an important indicator for predicting the diagnosis and severity of a patient′s condition and is worthy of clinical application and promotion.
