摘要
目的了解安徽省来安县艾滋病病毒(Human immunodeficiency virus,HIV)感染者/艾滋病(acquired immune deficiency syndrome,AIDS)患者(简称HIV/AIDS)抗逆转录病毒治疗(antiretroviral therapy,ART)免疫学效果和影响因素,为成人艾滋病抗病毒治疗提供依据。方法收集2007—2021年来安县的HIV/AIDS数据,用卡片编号将“中国疾病预防控制信息系统”随访库和抗病毒治疗库进行链接,采用Wilcoxon符号秩检验和多因素logistic回归进行分析。结果来安县HIV/AIDS治疗后第1年的CD4^(+)T淋巴细胞数较基线增长最多,增长值中位数为94.50(14.00~206.75)个/μl(Z=-6.146,P<0.05);启动ART后1年内免疫学治疗有效率为62.96%;多因素logistic回归分析显示,基线CD4^(+)T淋巴细胞数是免疫学治疗有效的影响因素。结论2007—2021年来安县艾滋病抗病毒治疗免疫学效果较好,治疗后第1年CD4^(+)T淋巴细胞数增长较快,基线CD4^(+)T淋巴细胞数是来安县艾滋病抗病毒治疗免疫学效果的影响因素,后期需提高艾滋病患者的服药依从性,提升患者随访管理工作质量。
Objective To understand the immunological effects and influencing factors of antiretroviral therapy(ART)in patients with HIV/AIDS in Lai′an County,Anhui Province,so as to provide basis for antiviral therapy for adults.Methods The HIV/AIDS data of Lai′an County from 2007 to 2021 were collected,and the follow-up database and antiviral treatment database of“China Disease Prevention and Control Information system”were linked with card number.Wilcoxon symbolic rank test and multivariate logistic regression were used for analysis.Results The number of CD4^(+)T lymphocytes in the first year after HIV/AIDS treatment in Lai′an County increased the most compared with the baseline,with a median increase of 94.50(14.00 to 206.75)piece/μl(Z=-6.146,P<0.05).The effective rate of immunological therapy within one year after starting ART was 62.96%.Multivariate logistic regression analysis showed that the number of baseline CD4^(+)T lymphocytes was a factor affecting the effectiveness of immunological therapy.Conclusion The immunological effect of antiviral therapy for AIDS in Lai′an County is better from 2007 to 2021,and the number of CD4^(+)T lymphocytes increased rapidly in the first year after treatment.The baseline CD4^(+)T lymphocyte count is the factor affecting the immunological effect of antiviral therapy in Lai′an County.In the later stage,it is necessary to improve the medication compliance of AIDS patients and improve the quality of patient follow-up management.
