慢性前列腺炎/慢性盆腔疼痛综合征对精液参数的影响

Effects of chronic prostatitis/chronic pelvic pain syndrome on semen parameters

目的探讨和观察慢性前列腺炎/慢性盆腔疼痛综合征(CP/CPPS)对精液质量的影响及其临床意义。方法回顾性选取2021年8月至2022年7月首都医科大学附属北京妇产医院生殖男科门诊伴CP/CPPS症状患者188例,根据是否有病原体感染[包括前列腺小体外泄蛋白(PSEP)、解脲脲原体、沙眼衣原体、细菌培养、白色念珠菌、淋球菌等]进行诊断分类,其中诊断有病原体感染ⅢA-CP组患者52例,无病原体感染ⅢB-CP组患者136例。选取健康男性80例作为对照组。采集所有受试者精液进行检查,包括液化时间、精液量、pH值、精子浓度、活力、形态及精子碎片率(DFI)等。结果与对照组比较,ⅢA-CP组精液量增多、pH值下降、液化时间延长、精子前向活率降低及DFI升高,差异均有统计学意义(P<0.05);与对照组比较,ⅢB组液化时间延长、精子前向活率下降及DFI升高,差异均有统计学意义(P<0.05)。结论ⅢA-CP对精液参数影响较大。不育患者应积极查找病因并治疗,提高精液质量以降低不育影响。

Objective To explore and observe the impact of chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS)on semen quality and its clinical significance.Methods From August 2021 to July 2022,188 CP/CPPS patients were retrospectively selected from the infertility clinic of Beijing Obstetrics and Gynecology Hospital,Capital Medical University,whose data such as prostate body efflux protein(PSEP),ureaplasma urealyticum,chlamydia trachomatis,bacterial,white bloodcells,gonorrhea and other substances were collected for diagnostic classification.There were 52 cases inⅢA-CP group and 136 cases inⅢB-CP group.Eighty normal cases were selected as the control group for comparison.All subjects'semen was collected for examination,including liquefaction time,semen volume,pH value,sperm concentration,motility,morphology,and sperm fragmentation rate(DFI).Results Compare with the control group,ⅢA-CP group had significant statistical differences in more semen volume,decreased pH,prolonged liquefaction time,and decreased PR and lower DFI,the differences were statistically significant(P<0.05).Compare with the control group,ⅢB-CP group had significant statistical differences in prolonged liquefaction time,and decreased PR and lower DFI,the differences were statistically significant(P<0.05).ConclusionⅢA-CP disease can decrease semen parameters and infertile men withⅢA-CP should be treated actively to reduce the risk of infertility.
