冠心病患者尿微量白蛋白、脂联素水平及其与冠脉狭窄程度的相关性研究

Correlation of Urinary Microalbumin and Adiponectin Levels with Coronary Artery Stenosis in Patients with Coronary Heart Disease

摘要 目的探讨冠心病(CHD)患者尿微量白蛋白、脂联素水平及其与冠脉狭窄程度的相关性。方法选取2020年10月至2021年10月郑州市中心医院收治的120例CHD患者为CHD组,所有患者均行冠状动脉造影术(CAG)检查,根据冠脉病变评分分为轻度组、中度组和重度组,随机选取同期120例健康体检人员为对照组。比较CHD患者和对照组尿微量白蛋白(MAU)、脂联素(APN)和血脂水平。采用Pearson相关性分析血脂、尿微量白蛋白与脂联素和冠脉狭窄程度的相关性。结果CHD组患者血清总胆固醇(TC)、甘油三酯(TG)和低密度脂蛋白胆固醇(LDL-C)水平均明显高于对照组,高密度脂蛋白胆固醇(HDL-C)水平明显低于对照组;CHD组患者MAU和APN水平均明显高于对照组;且随着冠心病患者冠脉狭窄程度增加,MAU和APN水平显著升高;Pearson相关性分析显示,CHD患者Gensini积分和血脂、MAU、APN水平均呈正相关,差异有统计学意义(P<0.05)。结论冠心病患者尿微量白蛋白、脂联素水平显著升高,且其升高水平与冠脉狭窄程度呈正相关。 Objective To investigate the levels of urinary microalbumin and adiponectin in patients with coronary heart disease(CHD)and their correlation with the degree of coronary artery stenosis.Methods A total of 120 patients with CHD admitted to Zhengzhou Central Hospital from October 2020 to October 2021 were selected as the CHD group.All patients underwent coronary angiography(CAG)examination and were divided into mild group,moderate group,and severe group according to coronary artery lesion score.In the same period,120 healthy people were randomly selected as the control group.The levels of microalbuminuria(MAU),adiponectin(APN),and blood lipids were compared between CHD patients and controls.Pearson correlation analysis was used to analyze the correlation between blood lipid,urine microalbumin,adiponectin,and the degree of coronary artery stenosis.Results Serum total cholesterol(TC),triglyceride(TG)and low-density lipoprotein cholesterol(LDL-C)levels in the CHD group were significantly higher than those in the control group,but high-density lipoprotein cholesterol(HDL-C)levels were significantly lower than those in the control group.The MAU and APN levels in the CHD group were significantly higher than those in the control group.The levels of MAU and APN increased significantly with the increasing degree of coronary artery stenosis in patients with coronary heart disease.Pearson correlation analysis showed that Gensini score was positively correlated with lipid,MAU,and APN levels in CHD patients,with statistical significance(P<0.05).Conclusion The levels of urinary microalbumin and adiponectin were significantly increased in patients with coronary artery disease,and the increased levels were positively correlated with the degree of coronary artery stenosis.
