富血小板纤维蛋白在拔牙后即刻种植术患者中的应用效果

Application effects of platelet-rich fibrin in patients with immediate implantation after tooth extraction

目的:观察富血小板纤维蛋白在拔牙后即刻种植术患者中的应用效果。方法:选取2020年2月至2022年4月该院收治的80例牙齿缺损患者进行前瞻性研究,按照随机数字表法将其分为对照组和研究组各40例。对照组实施拔牙后即刻种植术治疗,研究组在对照组基础上联合富血小板纤维蛋白治疗,比较两组手术前后美学效果[红色美学指数(PES)、白色美学指数(WES)评分]及术前、术后1周炎性因子[肿瘤坏死因子-α(TNF-α)、核因子κB(NF-κB)]水平。结果:术后3个月,研究组近中龈乳头、远中龈乳头、唇侧龈缘最高点位置、唇侧龈缘曲线、根面凸度、软组织颜色、软组织质地等各项PES评分均高于对照组,差异有统计学意义(P<0.05);研究组牙冠形态、牙冠外形轮廓、牙冠颜色、牙冠质地、牙冠透明度等WES各项评分均高于对照组,差异有统计学意义(P<0.05);术后1周,研究组血清TNF-α、NF-κB水平均低于对照组,差异有统计学意义(P<0.05)。结论:富血小板纤维蛋白辅助拔牙后即刻种植术治疗牙齿缺损患者,可提高其牙美学指数评分,降低血清TNF-α、NF-κB水平,效果优于单纯拔牙后即刻种植术治疗。

Objective:To observe application effects of platelet-rich fibrin in patients with immediate implantation after tooth extraction.Methods:A prospective study was conducted on 80 patients with dentition defects admitted to the hospital from February 2020 to April 2022.According to the random number table method,they were divided into control group and study group,40 cases in each group.The control group was treated with immediate implantation after tooth extraction,while the study group was treated with platelet-rich fibrin on the basis of that of the control group.The aesthetic effect[red aesthetic index(PES)and white aesthetic index(WES)scores]and the levels of inflammatory factors[tumor necrosis factor-α(TNF-α),nuclear factor-κB(NF-κB)]were compared between the two groups before and after the operation.Results:3 months after the operation,the PES scores of mesial gingival papilla,distal gingival papilla,the highest point of labial gingival margin,labial gingival margin curve,root convexity,soft tissue color and soft tissue texture in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).The WES scores of crown morphology,crown contour,crown color,crown texture and crown transparency in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).1 week after the operation,the serum TNF-αand NF-κB levels in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Platelet-rich fibrin assisted immediate implantation after tooth extraction can improve the dental aesthetic index scores and reduce the levels of serum TNF-αand NF-κB in the patients with dentition defects.Moreover,it is superior to single immediate implantation after tooth extraction.
