血清cTnI、CK-MB、CRP水平与不稳定型心绞痛患者心肌微循环的相关性

摘要 目的:分析血清肌钙蛋白I(cTnI)、肌酸激酶同工酶MB(CK-MB)、C反应蛋白(CRP)水平与不稳定型心绞痛(UA)患者心肌微循环的相关性。方法:回顾性分析2019年5月至2021年5月该院收治的124例心绞痛患者的临床资料,按照世界卫生组织(WHO)分级标准将心绞痛患者分为稳定型心绞痛(SA)组(n=52)和UA组(n=72),另选取同期50名健康体检者为对照组,比较三组cTnI、CK-MB以及CRP水平。将UA组按照Braunwald分级分为低危险组(n=22)、中危险组(n=24)及高危险组(n=26),比较三组cTnI、CK-MB、CRP水平和心肌微循环状态,并采用Pearson相关性分析UA患者的cTnI、CK-MB、CRP水平与心肌微循环状态的相关性。结果:UA组、SA组的cTnI、CK-MB、CRP水平均高于对照组,且UA组高于SA组,差异均有统计学意义(P<0.05);高危险组、中危险组的cTnI、CK-MB、CRP水平均高于低危险组,且高危险组高于中危险组,差异均有统计学意义(P<0.05);高危险组、中危险组的左前降支(LAD)、左回旋支(LCX)及右冠状动脉(RCA)的校正的TIMI血流计数帧数(CTFC)均高于低危险组,且高危险组高于中危险组,差异均有统计学意义(P<0.05);Pearson相关性分析结果显示,UA患者的cTnI、CK-MB、CRP水平与LAD CTFC、LCX CTFC、RCA CTFC均呈正相关(r>0,P<0.05)。结论:UA患者的cTnI、CK-MB、CRP水平与心肌微循环状态呈正相关。 Objective:To analyze correlations among serum troponin I(cTnI),creatine kinase isoenzyme MB(CK-MB),C-reactive protein(CRP)levels and myocardial microcirculation in patients with unstable angina(UA).Methods:The clinical data of 124 patients with angina pectoris admitted to the hospital from May 2019 to May 2021 were retrospectively analyzed.According to the World Health Organization(WHO)classification criteria,the patients with angina pectoris were divided into stable angina(SA)group(n=52)and UA group(n=72).Another 50 healthy subjects were selected as control group.The levels of cTnI,CK-MB and CRP were compared among the three groups.The UA group was subdivided into low risk group(n=22),medium risk group(n=24)and high risk group(n=26)according to the Braunwald classification.The levels of cTnI,CKMB,CRP and myocardial microcirculation were compared among the three groups.Pearson correlation was used to analyze the correlation among the cTnI,CK-MB,CRP levels and myocardial microcirculation status in the UA patients.Results:The levels of cTnI,CK-MB and CRP in the SA group and the UA group were higher than those in control group,those in the UA group was higher than those in the SA group,and the differences were statistically significant(P<0.05).The levels of cTnI,CK-MB and CRP in the high risk group and the medium risk group were higher than those in the low risk group,those in the high risk group were higher than those in the medium risk group,and the differences were statistically significant(P<0.05).The corrected TIMI frame count(CTFC)levels of left anterior descending(LAD),left circumflex(LCX)and right coronary artery(RCA)in the medium risk group and the high risk group were higher than those in the low risk group,those in the high risk group were higher than those in the medium risk group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the levels of cTnI,CKMB and CRP in the UA patients were positively correlated with LAD CTFC,LCX CTFC and RCA CTFC(r<1,P<0.05).Conclusions:The levels of cTnI,CK-MB and CRP in the UA patients are positively correlated with myocardial microcirculation.
