超声SWE联合RTE在乳腺良恶性肿块诊断中的效能

摘要 目的:分析超声实时剪切波弹性成像技术(SWE)联合实时组织弹性成像技术(RTE)在乳腺良恶性肿块诊断中的效能。方法:选取2018年1月至2021年7月该院收治的70例乳腺肿块患者进行前瞻性研究。所有患者均行超声SWE联合RTE检测,以病理穿刺活检结果作为“金标准”,比较超声SWE、RTE检测在乳腺肿块良恶性诊断中的准确度、特异度、灵敏度,采用Kappa检验评估超声SWE检测、RTE检测及联合诊断结果与病理穿刺活检诊断乳腺肿块良恶性的一致性。结果:金标准结果显示,70例乳腺肿块患者中恶性31例,良性39例;超声SWE联合RTE检测诊断乳腺良恶性肿块的准确度、特异度均高于超声SWE、RTE单项检查,差异均有统计学意义(P<0.05);超声SWE、RTE单项及联合检测诊断乳腺良恶性肿块的灵敏度比较,差异均无统计学意义(P>0.05);经Kappa一致性检验,超声SWE、RTE单项检测与病理穿刺活检诊断乳腺良恶性肿块一致性一般(Kappa=0.432、0.516,P<0.05);超声SWE联合RTE检测与病理穿刺活检诊断乳腺良恶性肿块一致性较好(Kappa=0.827,P<0.05)。结论:超声SWE检测联合RTE检测在乳腺良恶性肿块诊断中的效能高于二者单项检测。 Objective:To analyze efficacy of real-time shear wave elastography(SWE)combined with real-time tissue elastography(RTE)in diagnosis of benign and malignant breast masses.Methods:A prospective study was conducted on 70 patients with breast masses admitted to the hospital from January 2018 to July 2021.All patients underwent ultrasound SWE combined with RTE detection.Using the results of pathological biopsy as the‘gold standard’,the accuracy,specificity and sensitivity of SWE and RTE in the diagnosis of benign and malignant breast masses were compared.Kappa test was used to evaluate the consistencies of ultrasound SWE detection,RTE detection and combined diagnosis with pathological biopsy in the diagnosis of benign and malignant breast masses.Results:The gold standard showed that among the 70 patients with breast masses,31 were malignant and 39 were benign.The accuracy and the specificity of ultrasound SWE combined with RTE in the diagnosis of benign and malignant breast masses were higher than those of single ultrasound SWE detection and single RTE detection,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the sensitivity among the single and combined detection of SWE and RTE in the diagnosis of benign and malignant breast masses(P>0.05).Kappa consistency test showed that the consistency of single detection of ultrasound SWE and RTE with pathological biopsy in the diagnosis of benign and malignant breast masses was fair(Kappa=0.432,0.516,P<0.05).However,ultrasound SWE combined with RTE had a good consistency with pathological biopsy in the diagnosis of benign and malignant breast masses(Kappa=0.827,P<0.05).Conclusions:The diagnostic efficiency of ultrasound SWE combined with RTE in the diagnosis of benign and malignant breast masses is higher than that of the single detection.
