摘要
目的研究HBV与HPV对宫颈上皮内病变及宫颈癌的影响,探讨HBV感染与宫颈上皮内病变及HPV感染的关系。方法选取东莞东华医院2014年至2018年首次进行宫颈组织病理学、HPV分型及HBV-DNA检查的患者,用显微镜观察有无宫颈上皮内病变,流式荧光杂交技术检测27种HPV亚型,实时荧光定量PCR技术检测HBV-DNA载量,统计HPV感染及其亚型感染数量对宫颈上皮内病变的影响,并统计HBV-DNA载量大于1000时对HPV感染情况及宫颈上皮内病变的影响。结果在1166例进行HPV分型和宫颈组织病理学检查的患者中,与HPV阴性组相比,HPV阳性组对宫颈上皮内病变及宫颈癌影响差异有显著统计学意义(P<0.01);在546例HPV分型检测阳性患者中,与单纯感染组相比,多重感染组对宫颈上皮内病变及宫颈癌影响差异无统计学意义(P>0.05);在126例进行HBV-DNA载量和HPV分型检测患者中,与HBV-DNA载量≤1000组相比,HBV-DNA载量>1000组中HPV感染率差异无统计学意义(P>0.05);在140例进行HBV-DNA载量和宫颈组织病理学检查患者中,与HBV-DNA载量≤1000组相比,HBV-DNA载量>1000组对宫颈上皮内病变及宫颈癌影响差异无统计学意义(P>0.05)。结论感染HPV会明显促进宫颈癌及其癌前病变发生,但感染HPV亚型数量对宫颈上皮内病变没有影响;HBV感染与HPV感染无关,感染HBV对宫颈上皮内病变及宫颈癌没有影响,为以后研究宫颈癌相关新的影响因素打下基础。
Objective To study the effects of Hepatitis B virus(HBV)and human papillomavirus(HPV)on cervical intraepithelial lesions and cervical cancer,and to explore the relationship between HBV infectin and cervical intraepithelial lesions and HPV infection.Methods Patients who underwent cervical histopathology,HPV typing and HBV-DNA test for the first time in DongGuan Tungwah Hospital from 2014 to 2018 were selected.Cervical intraepithelial lesions were observed by microscope.27 HPV subtypes were detected by flow fluorescence hybridization,and HBV-DNA load was detected by real-time fluorescent quantitative PCR.The impact of HPV infection and the number of its subtypes on cervical intraepithelial lessons ws counted,and the impact of HBV DNA load greater than 1 O0O on HPV infection and cervical intraepithelial lesions was counted.Results Among 1166 patients who underwent both HPV typing and cervi-cal histomathology,compared with the HPV-negative group,the HPV-positive group has statistically significant differences in the influence of cervical intraepithelial lesions and cervical cancer(P<0.01).Among the 546 patients with positive HPV typing test,the effect of multiple infections on cervical intraepithelial lesions and cervical cancer was not statistically significant(P>0.05),compared with that of single infection.Among the 126 patients who underwent HBVDNA load and HPV typing tests,compared with the HBV-DNA load<1 o00 group,the differential HPV infection rate in the HBV-DNA load>1000 group was not statistically significant(P>0.05).Among the 140 patients who underwent HBV-DNA load and cervical histomathology,compared with the HBV-DNA load≤1000 group,there was no significant difference in the effect of HBV-DNA load>1 o00 group on cervical intraepithelial lesions and cervical cancer(P>0.05).Conclusion The infection of HPV can significantly increase the incidence of cervical cancer and basal precancerous lesions,but the number of infected HPV subtypes has no significant effect on cervical intraepithelial disease.HBV infection is not related to HPV infection,and HBV infection has no effect on cervical intraepithelial lesions and cervical cancer,which lay a foundation for the future research on new influencing factors related to cervical cancer.
