乳腺癌相关淋巴水肿患者肢体功能-残疾-健康现状及影响因素

Upper extremity function,disability and health among patients with breast cancer-related lymphedema:the influencing factors

目的调查分析乳腺癌相关淋巴水肿患者肢体功能、心理、家务活动、移动性活动及社会交往现状及其影响因素。方法采用中文版淋巴水肿肢体功能-残疾-健康量表、疾病感知问卷简化版、癌症患者生活质量核心量表对132例乳腺癌相关淋巴水肿患者进行调查。结果乳腺癌相关淋巴水肿患者肢体功能-残疾-健康总分为(39.43±15.37)分,82.5%患者的肢体功能-残疾-健康状况处于中等及以上严重程度。多元线性回归分析显示,躯体功能、认知、淋巴水肿持续时间、年龄以及淋巴水肿分期是乳腺癌相关淋巴水肿患者肢体功能-残疾-健康现状的主要影响因素(均P<0.05)。结论乳腺癌相关淋巴水肿患者存在着中等及以上严重程度的上肢功能障碍、心理问题、家务活动障碍、移动性活动障碍及社会交往问题,建议医护人员基于循证理念采取最佳的针对性措施减轻水肿程度,改善其肢体功能与健康状况,提高其自我管理能力和健康生活质量。

Objective To investigate the upper limb function,disability and health status in a group of patients with breast cancer-related lymphedema,and to analyze its influencing factors.Methods A survey was conducted on 132 patients with breast cancer-related lymphedema were evaluated using the Lymph-ICF-UL questionnaire,the Brief Illness Perception Questionnaire and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30.Results The mean total score of Lymph-ICF-UL questionnaire was 39.43±15.37,and 82.5%of them reported a moderate or above amount of problems in general functioning,disability and health.Multiple linear regression analysis revealed that physical function,cognition,duration of lymphedema,age and lymphedema stage were main influencing factors.Conclusion The presence of breast cancer-related lymphedema had a serious impact on the physical functioning,mental functioning,household activities,mobility activities,and life and social activities.It suggestes that optimize health promoting strategies should be provided for them so as to relieve lymphedema,improve the level of functioning disability and health status,self-management ability and enhance their quality of life.
