程序化健康教育联合心理干预在足月胎膜早破产妇中的应用效果分析 被引量:4

摘要 目的 探究程序化健康教育联合心理干预对足月胎膜早破产妇的效果。方法 120例足月胎膜早破的产妇,按随机数字表法分为观察组与对照组,各60例。对照组产妇给予常规心理干预,观察组产妇在对照组的基础上给予程序化健康教育。比较两组分娩方式(自然分娩、剖宫产、引产)、干预前后的焦虑自评量表(SAS)评分、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、细胞间粘附分子-1(ICAM-1)、产后并发症发生情况、产后满意度。结果 观察组自然分娩率、剖宫产率分别为58.33%、33.33%,高于对照组的33.33%、16.67%,引产率8.33%低于对照组的50.00%,差异均有统计学意义(P<0.05)。干预后,两组SAS评分低于干预前,且观察组SAS评分(48.07±2.83)分低于对照组的(57.92±2.55)分,差异有统计学意义(P<0.05)。干预后,两组IL-6、TNF-α、ICAM-1均低于干预前,且观察组TNF-α、IL-6、ICAM-1分别为(49.99±13.02)pg/ml、(60.92±13.92)pg/ml、(216.63±4.92)μg/L,低于对照组的(60.03±11.92)pg/ml、(74.82±12.07)pg/ml、(238.92±2.85)μg/L,差异有统计学意义(P<0.05)。观察组产后并发症发生率11.67%,低于对照组的26.67%,差异有统计学意义(P<0.05)。观察组产后满意度91.67%高于对照组的73.33%,差异有统计学意义(P<0.05)。结论 程序化健康教育联合心理干预能够明显减轻产妇焦虑情绪,提高自然分娩率,降低产妇机体炎症应激及并发症的发生,提高产妇及其家属的满意度,值得在临床借鉴应用。 Objective To investigate the effect of programmed health education combined with psychological intervention in pregnant women with premature rupture of membranes at term.Methods A total of 120 pregnant women with premature rupture of membranes at term were divided into observation group and control group according to the random numerical table,with 60 cases in each group.The control group received conventional psychological intervention,and the observation group received programmed health education on the basis of the control group.Both groups were compared in terms of mode of delivery(natural delivery,cesarean section,induced labor),self-rating anxiety scale(SAS)score,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),intercellular adhesion molecule-1(ICAM-1)before and after intervention,postpartum complications,and postpartum satisfaction.Results The rates of natural delivery and cesarean section in the observation group were 58.33%and 33.33%,which were higher than those of 33.33%and 16.67%in the control group;the rate of induced labor of 8.33%in the observation group was lower than that of 50.00%in the control group;the differences were statistically significant(P<0.05).After intervention,SAS score in the two groups was lower than that before intervention in this group,and SAS score of(48.07±2.83)points in the observation group was lower than that of(57.92±2.55)points in the control group.The differences were statistically significant(P<0.05).After intervention,IL-6,TNF-αand ICAM-1 in both groups were lower than those before intervention in this group;IL-6,TNF-αand ICAM-1 were(49.99±13.02)pg/ml,(60.92±13.92)pg/ml and(216.63±4.92)μg/L in the observation group,which were lower than those of(60.03±11.92)pg/ml,(74.82±12.07)pg/ml and(238.92±2.85)μg/L in the control group;the differences were statistically significant(P<0.05).The incidence of postpartum complications was 11.67%in the observation group,which was lower than that of 26.67%in the control group,and the difference was statistically significant(P<0.05).The postpartum satisfaction was 91.67%in the observation group,which was higher than that of 73.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion Programmed health education combined with psychological intervention can significantly reduce maternal anxiety, improve the rate of natural delivery, reduce the occurrence of maternal inflammatory stress and complications, and improve the satisfaction of maternal and their families, which is worthy of clinical reference and application.
