全方位干预结合提肛运动在低位直肠癌保肛术后患者中的应用效果分析

Analysis of the application effect of all-round intervention combined with lift exercise in patients with low rectal cancer after anal sphincter preserving operation

目的 探讨全方位干预结合提肛运动在低位直肠癌保肛术后患者中的应用效果.方法 将98例低位直肠癌患者根据干预方案的不同分为对照组52例(提肛运动)和观察组46例(提肛运动+全方位干预).比较两组患者肛门功能指标、排便功能评分、切口疼痛情况[视觉模拟评分法(VAS)]、生活质量[欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)].结果 观察组患者肛门最大静息压、肛门最大收缩压、肛门收缩向量容积均明显大于对照组,肛门功能良好率明显高于对照组,差异均有统计学意义(P﹤0.01).干预后,两组患者排便功能评分及VAS评分均较干预前降低,且观察组患者排便功能评分及VAS评分均低于对照组,差异均有统计学意义(P﹤0.05).干预后,两组患者躯体功能、角色功能、情绪功能、睡眠质量评分均较干预前升高,且观察组患者躯体功能、角色功能、情绪功能、睡眠质量评分均高于对照组,差异均有统计学意义(P﹤0.05).结论 低位直肠癌保肛术后予以提肛运动+全方位干预能改善患者肛门功能及生活质量,干预效果理想.

Objective To investigate the application effect of all-round intervention combined with lift exercise in anal sphincter preserving operation.Method A total of 98 low rectal cancer patients were divided into control group(lift exercise)and observation group(lift exercise+all-round intervention)according to different intervention programs.The anal function indicators,defecation function scores,incision pain[visual analogue scale(VAS)],and quality of life[European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)]were compared between the two groups.Result The maximum resting pressure of the anus,the maximum systolic blood pressure of the anus,and the volume of anal contraction vector in the observation group were significantly higher than those in the control group,and the rate of anal normal function was significantly higher than that in the control group,and the differences were statistically significant(P<0.01).After the intervention,the defecation function score and VAS score of the two groups were lower than those before the intervention,and the above scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the intervention,the scores of somatic function,role function,emotional function,and sleep quality of the two groups were higher than those before the intervention,and the scores of the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Lift exercise plus all-round intervention after anal sphincter preserving operation for low rectal cancer could improve the patient’s anal function and quality of life,and the intervention effect could meet the requirement.
