前庭康复治疗前庭性偏头痛的临床效果

Clinical effect of vestibular rehabilitation on vestibular migraine

目的探讨前庭康复治疗前庭性偏头痛(VM)的临床效果。方法选取2018年9月至2020年9月重庆市第五人民医院神经内科收治的48例VM患者作为研究对象,按照随机数字表法分为治疗组与对照组,每组各24例。治疗组采用盐酸氟桂利嗪药物联合前庭康复(VR)治疗,对照组采用盐酸氟桂利嗪药物治疗。比较两组治疗前和治疗4周后的眩晕障碍量表(DHI)、健康调查简表(SF-36)、汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分。结果治疗前,两组患者DHI、HAMA和HAMD评分比较,差异无统计学意义(P>0.05);治疗后,两组患者DHI、HAMA和HAMD评分均低于治疗前,且治疗组患者DHI、HAMA评分均低于对照组,差异有统计学意义(P<0.05),而两组间HAMD评分比较,差异无统计学意义(P>0.05)。治疗后,治疗组生活质量的生理职能、情感职能评分高于对照组,差异有统计学意义(P<0.05)。结论前庭康复治疗改善了VM患者的生活质量和心境状况。

Objective To investigate the clinical effect of vestibular rehabilitation for vestibular migraine(VM).Methods A total of 48 VM patients admitted to the Department of Neurology,the Fifth People's Hospital of Chongqing from September 2018 to September 2020 were selected as research objects and divided into treatment group and control group according to random number table method,with 24 patients in each group.The treatment group was treated with Flunarizine Hydrochloride combined with vestibular rehabilitation(VR),and the control group was treated with Flunarizine Hydrochloride.The scores of dizziness disorder inventory(DHI),short form of health survey(SF-36),Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)were compared between the two groups before and after 4 weeks of treatment.Results Before treatment,there were no statistical significances in DHI,HAMA and HAMD scores between the two groups(P>0.05).After treatment,DHI,HAMA and HAMD scores of the two groups were lower than before,and DHI and HAMA scores of the treatment group were lower than those of the control group,with statistical significance(P<0.05),while HAMD scores between the two groups showed no statistical significance(P>0.05).After treatment,the scores of physiological function and emotional function of quality of life in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Vestibular rehabilitation therapy improved the quality of life and psychological status of VM patients.
