人参葛根方治疗结膜松弛症脾肺气虚证的临床观察

Clinical observation of Ginseng Gegen Prescription in treating conjunctivochalasis Spleen-lung Qi deficiency

目的观察人参葛根方治疗结膜松弛症脾肺气虚证的效果。方法选取2017年1月至12月于上海中医药大学附属普陀医院眼科诊治的73例中医辨证为“脾肺气虚”证的结膜松弛症患者作为研究对象,采用随机数字表法将其分为治疗组(36例)和对照组(37例)。治疗组口服人参葛根方,对照组采用玻璃酸钠滴眼液。治疗2个月后比较两组患者治疗前后的泪河面积(TMA)、中医证候积分、眼表疾病指数(OSDI)、泪膜破裂时间(BUT)、基础泪液分泌试验(SⅠT)。结果治疗后,治疗组的中央点TMA高于对照组,差异有统计学意义(P<0.05),两组的鼻侧点和颞侧点的TMA比较,差异无统计学意义(P>0.05)。治疗后,治疗组中医证候积分、OSDI低于对照组,治疗组BUT长于对照组,差异有统计学意义(P<0.05);两组的SⅠT比较,差异无统计学意义(P>0.05)。治疗前后的OSDI和中央点TMA均呈负相关(P<0.05)。结论人参葛根方可以作为结膜松弛症脾肺气虚证的一种有效的中医药替代疗法。

Objective To observe the effect of Ginseng Gegen Prescription in treating conjunctivochalasis Spleen-lung Qi deficiency.Methods From January to December 2017,73 patients with conjunctivochalasia who were diagnosed as"Spleen-lung Qi deficiency"syndrome in traditional Chinese medicine during the period of outpatient treatment or inpatient treatment in the Department of Ophthalmology,Putuo Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were selected as study subjects,and were divided into treatment group(36 cases)and control group(37 cases)by random number table method.Patients in the treatment group were treated with Ginseng Gegen Prescription,while patients in the control group were treated with Sodium Hyaluronate Eye Drops.After 2 months of treatment,the tear meniscus area(TMA),traditional Chinese medicine(TCM)symptom score,ocular surface disease index(OSDI),tear break-up time(BUT),SchirmerⅠtest(SⅠT)of the two groups were compared before and after treatment.Results After treatment,the TMA of the central point in the treatment group was higher than that of the control group,with statistical significance(P<0.05),but there was no statistical significance in the TMA of the nasal point and the temporal point between the two groups(P>0.05).After treatment,TCM syndrome score and OSDI in the treatment group were lower than those in the control group,BUT of the treatment group was longer than the control group,the differences were statistically significant(P<0.05).There was no significant difference in SⅠT between the two groups(P>0.05).OSDI and central point TMA were negatively correlated before and after treatment(P<0.05).Conclusion Ginseng Gegen Prescription can be used as an effective alternative therapy of traditional Chinese medicine for Spleen-lung Qi deficiency syndrome in conjunctivochalasis.
