卒中后尿失禁患者应用盆底肌电刺激联合Kegel运动的康复效果观察

Observation on rehabilitation effect of pelvic floor electromyoelectric stimulation combined with Kegel exercise in patients with urinary incontinence after stroke

目的 观察盆底肌电刺激联合凯格尔(Kegel)运动对脑卒中后尿失禁患者的护理效果。方法 选择2021年7月至2022年7月在新疆医科大学第二附属医院经颅脑CT或MRI检查确诊的脑卒中后尿失禁患者100例,采用随机数字表法分为对照组和观察组,各50例。对照组采用常规护理,观察组在常规护理基础上采用盆底肌电刺激联合Kegel运动,比较两组治疗前后24 h排尿次数、24 h尿失禁次数、盆底肌表面肌电评估(Glazer)总得分及尿失禁生活质量问卷(I-QOL)评分。结果 治疗后,与对照组比较,观察组24 h排尿次数、24 h尿失禁次数减少,差异有统计学意义(P<0.05),Glazer总得分、I-QOL得分增高,差异有统计学意义(P<0.05)。结论 盆底肌电刺激联合Kegel运动能够改善脑卒中后尿失禁患者的尿失禁症状,提高患者生活质量,具有临床推广价值。

Objective To observe the nursing effect of electrical stimulation of pelvic floor muscles combined with Kegel exercise on patients with urinary incontinence after stroke.Methods 100 patients with post-stroke urinary incontinence diagnosed through brain CT or MRI examination in the hospital from July 2021 to July 2022 were selected and randomly divided into control group and observation group,with 50 patients in each group.The control group was given routine nursing,and the observation group was given pelvic floor electromyography combined with Kegel exercise on the basis of routine nursing.The 24 h frequency of urination,24 h frequency of urinary incontinence,total pelvic floor surface electromyography assessment(Glazer)score and urinary incontinence quality of life questionnaire(I-QOL)score before and after the treatment were compared between the two groups.Results After the treatment,compared withcontrol group,24 h urination times and 24 h urinary incontinence times in observation group were reduced,and the difference was statistically significant(P<0.05).The Glazer′s total score and I-QOL score were increased,and the difference was statistically significant(P<0.05).Conclusion Pelvic floor electrical muscle stimulation combined with Kegel exercise can improve the symptoms of urinary incontinence in patients with post-stroke urinary incontinence,improve the quality of life of the patients,and it has the value of clinical promotion.
