四磨汤联合乳果糖口服溶液、双歧杆菌三联活菌肠溶胶囊治疗老年糖尿病便秘临床观察

Clinical efficacy of Simo Decoction combined with lactulose,bifidobacterium triple viable bacteria on elderly diabetes patients with constipation

目的 观察四磨汤联合乳果糖口服溶液、双歧杆菌三联活菌肠溶胶囊治疗老年糖尿病便秘的临床疗效。方法 将101例老年糖尿病便秘患者按照随机数字表法分为2组,对照组51例予乳果糖口服溶液、双歧杆菌三联活菌肠溶胶囊治疗,治疗组50例在对照组治疗基础上联合四磨汤治疗。2组均治疗2周。比较2组临床疗效;比较2组治疗前后中医证候评分变化;比较2组治疗前后血糖指标、肠道菌群水平变化。结果 治疗组总有效率90.00%(45/50),对照组总有效率74.51%(38/51),治疗组疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后FPG、2 hPG均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后大肠杆菌水平均降低(P<0.05),乳酸杆菌、双歧杆菌水平均升高(P<0.05);治疗后治疗组大肠杆菌水平均低于对照组(P<0.05),乳酸杆菌、双歧杆菌水平均高于对照组(P<0.05)。结论 四磨汤联合乳果糖口服溶液、双歧杆菌三联活菌肠溶胶囊治疗老年糖尿病便秘,可改善患者肠道菌群失调,控制血糖,提高临床疗效,且安全可靠。

Objective To observe the clinical efficacy of Simo Decoction combined with lactulose,bifidobacterium triple viable bacteria on elderly diabetes patients with constipation.Methods A total of 101 elderly diabetes patients with constipation were randomly assigned into the treatment group(n=50)and the control group(n=51).All patients were managed by lactulose oral solution and bifidobacterium triple viable bacteria,and those in the treatment group were additionally treated with Simo Decoction.Treated for 2 weeks,the aim was to compare traditional Chinese medicine(TCM)syndrome score,blood glucose index,intestinal flora.The curative effect was assessed.Results After treatment,the overall effective rate in the treatment group was better than that in the control group(90.00%[45/50]vs 74.51%[38/51],[P<0.05],respectively).TCM symptom scores in groups were decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).The fasting plasma glucose(FPG),2-hour postprandial blood glucose(2 hPG)in groups were decreased(P<0.05),the treatment group was more prevalent(P<0.05).Decreased Escherichia coli,and increased Lactobacillus,Bifidobacterium were found in groups than those before treatment(all P<0.05),and the different was significant between groups for the above indexes(all P<0.05).Conclusion For elderly diabetes patients with constipation,the combinations can improve intestinal flora disorder and blood glucose control,clinical efficacy,and with safety and reliability.
