胸外科术后患者采取呼吸肌评估和呼吸训练指导的意义分析 被引量:1

摘要 目的分析胸外科术后患者采取呼吸肌评估和呼吸训练指导的意义。方法60例胸外科术后患者,随机分为对照组和试验组,各30例。对照组采取常规干预,试验组在对照组基础上采取呼吸肌评估和呼吸训练指导。比较两组患者肺功能指标[每分钟最大通气量(MVV)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)]、血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、血氧饱和度(SpO_(2))]、生活质量评分。结果试验组患者MVV(99.21±5.12)L/min、FVC(3.16±0.70)L、FEV1(2.72±0.50)L均高于对照组的(92.13±4.72)L/min、(2.76±0.20)L、(2.22±0.40)L,差异有统计学意义(P<0.05)。试验组患者PaO_(2)(92.32±1.37)mm Hg(1 mm Hg=0.133 kPa)、SpO_(2)(99.20±1.10)%均高于对照组的(82.21±1.27)mm Hg、(97.70±0.90)%,PaCO_(2)(35.12±0.98)mm Hg低于对照组的(41.53±1.03)mm Hg,差异有统计学意义(P<0.05)。试验组患者生理机能、生理职能、躯体疼痛、一般健康状况、社会功能、情感职能评分分别为(71.0±3.5)、(66.5±7.1)、(61.3±5.1)、(64.2±6.2)、(66.2±7.2)、(65.3±6.2)分,均高于对照组的(62.6±2.5)、(61.4±4.5)、(57.8±4.9)、(58.2±5.8)、(60.7±6.0)、(61.1±5.1)分,差异有统计学意义(P<0.05)。结论胸外科术后患者采取呼吸肌评估和呼吸训练指导,可有效改善患者肺功能,提高其生活质量,值得推广。 Objective To analyze the significance of respiratory muscle assessment and respiratory training instruction for post-thoracic surgery patients.Methods A total of 60 cases of post-thoracic surgery patients were randomly divided into control group and experimental group,with 30 cases in each group.The control group received conventional intervention,and the experimental group received respiratory muscle assessment and respiratory training instruction based on the control group.Both groups were compared in terms of pulmonary function indexes[maximum ventilatory volume(MVV),forced vital capacity(FVC),forced expiratory volume in one second(FEV1)],blood gas analysis indexes[arterial partial pressure of oxygen(PaCO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygen saturation(SpO_(2))],and quality of life score.Results The experimental group had MVV of(99.21±5.12)L/min,FVC of(3.16±0.70)L,and FEV1 of(2.72±0.50)L,which were higher than those of(92.13±4.72)L/min,(2.76±0.20)L,and(2.22±0.40)L in the control group,and the differences were statistically significant(P<0.05).PaCO_(2)of(92.32±1.37)mm Hg(1 mm Hg=0.133 kPa)and SpO_(2) of(99.20±1.10)mm Hg in the experimental group were higher than those of(82.21±1.27)and(97.70±0.90)mm Hg in the control group;PaCO_(2)of(35.12±0.98)%in the experimental group was lower than that of(41.53±1.03)%in the control group;the differences were statistically significant(P<0.05).The scores of physical functioning,role-physical,bodily pain,general health status,social functioning and role-emotional were(71.0±3.5),(66.5±7.1),(61.3±5.1),(64.2±6.2),(66.2±7.2)and(65.3±6.2)points in the experimental group,which were higher than those of(62.6±2.5),(61.4±4.5),(57.8±4.9),(58.2±5.8),(60.7±6.0)and(61.1±5.1)points in the control group,and the differences were statistically significant(P<0.05).Conclusion Respiratory muscle assessment and respiratory training instruction for post-thoracic surgery patients can effectively improve the pulmonary function and quality of life of patients,and is worthy of promotion.
