摘要
目的探讨细胞周期蛋白A1(Cyclin A1,CCNA1)在甲状腺癌组织中的表达及其与甲状腺癌预后的关系。方法收集本院2016年1月至2017年9月期间收治的,术后确诊为甲状腺癌的104例患者的甲状腺癌组织。采用免疫组织化学染色法,检测患者甲状腺癌组织中CCNA1蛋白的表达情况。根据甲状腺癌组织CCNA1蛋白表达检测结果进行分组,将CCNA1蛋白表达阳性的病例设为阳性组(55例),将CCNA1蛋白表达阴性的病例设为阴性组(49例)。分析CCNA1蛋白表达与甲状腺癌临床病理特征的关系。比较阳性组、阴性组的无进展生存期(progression-free survival,PFS)、总生存期(overall survival,OS)。结果CCNA1蛋白表达情况与甲状腺癌患者的肿瘤直径、TNM分期、分化程度、淋巴结转移情况密切相关。肿瘤直径≥4 cm、TNM分期为III/IV期、分化程度低、有淋巴结转移的患者,CCNA1蛋白表达阳性的构成比更高(P<0.05)。总体5年生存率为90.38%。CCNA1蛋白表达阴性组、阳性组的中位OS分别为64.61个月、62.86个月,组间差异有统计学意义(χ^(2)=3.906,P=0.048)。CCNA1蛋白表达阴性组、阳性组的中位PFS分别为59.02个月、54.67个月,组间差异有统计学意义(χ^(2)=4.351,P=0.037)。结论CCNA1蛋白表达与甲状腺癌肿瘤直径、TNM分期、分化程度、淋巴结转移情况等均有密切关系,CCNA1蛋白表达水平高,则甲状腺癌患者预后较差。
Objective To investigate the expression of Cyclin A1(CCNA1)in thyroid cancer and its relationship with the prognosis.Methods Thyroid cancer tissues of 104 patients diagnosed as thyroid cancer after surgery admitted to our hospital from January 2016 to September 2017 were collected.Immunohistochemical staining was used to detect the expression of CCNA1 protein in thyroid cancer tissues.According to the detection results of CCNA1 protein expression in thyroid cancer tissues,the cases with positive CCNA1 protein expression were selected as the positive group(n=55),and the cases with negative CCNA1 protein expression were selected as the negative group(n=49).To analyze the relationship between CCNA1 protein expression and clinicopathological features of thyroid carcinoma.The progression-free survival(PFS)and overall survival(OS)were compared between the two groups.Results The expression of CCNA1 protein was closely related to the tumor diameter,TNM stage,differentiation degree and lymph node metastasis of thyroid cancer patients.Patients with tumor diameter≥4 cm,TNM stage III/IV,low differentiation and lymph node metastasis had a higher proportion of positive CCNA1 protein expression(P<0.05).The overall 5-year survival rate was 90.38%.The median OS of CCNA1 protein expression negative group and positive group were 64.61 months and 62.86 months,respectively,and the difference between the groups was statistically significant(χ^(2)=3.906,P=0.048).The median PFS of CCNA1 protein expression negative group and positive group were 59.02 months and 54.67 months,respectively,and the difference was statistically significant(χ^(2)=4.351,P=0.037).Conclusion The expression of CCNA1 protein is closely related to the tumor diameter,TNM stage,differentiation degree and lymph node metastasis of thyroid cancer.High expression level of CCNA1 protein is associated with poor prognosis of thyroid cancer patients.
