奥拉西坦、前列地尔、氯吡格雷三者联合治疗急性脑梗死的临床观察

Clinical observation of the effect of the combination of oxiracetam,alprostadil and clopidogrel in the treatment of acute cerebral infarction

目的观察奥拉西坦、前列地尔、氯吡格雷三者联合治疗急性脑梗死(AIS)的效果。方法135例AIS患者随机分为A、B、C组,每组45例。A组采用氯吡格雷联合奥拉西坦治疗;B组采用氯吡格雷联合前列地尔治疗;C组采用氯吡格雷、奥拉西坦、前列地尔三者联合治疗。对比3组的疗效、认知功能、炎症指标和不良反应发生情况。结果C组的疗效明显优于A组和B组,肿瘤坏死因子-α、超敏C反应蛋白、白介素-6水平均明显低于A组和B组,简易精神状态评定量表(MMSE)评分高于A组和B组(P<0.05或0.01)。3组不良反应发生率差异无统计学意义(P>0.05)。结论奥拉西坦、前列地尔、氯吡格雷三者联合治疗AIS可提高疗效,减轻炎症反应程度,改善认知功能,且不增加不良反应发生率。

Objective To observe the effect of the combination of oxiracetam,alprostadil and clopidogrel in the treatment of acute cerebral infarction(AIS).Methods A total of 135 AIS patients were randomized to Group A,B,and C,45 cases in each group.Group A was treated with clopidogrel combined with oxiracetam.Group B was treated with clopidogrel combined with alprostadil.Group C was treated with combination of clopidogrel,oxiracetam and alprostadil.The efficacy,cognitive function,inflammatory indicators and adverse reactions of the three groups were compared.Results The curative effect of Group C was significantly superior to that of Group A and Group B(P<0.05);The TNF-α,hs-CRP and IL-6 levels of Group C were significantly lower than those of Group A and Group B;The MMSE score of Group C was significantly higher than that of Group A and B(P<0.05 or 0.01).There was no significant difference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion The combination of oxiracetam,alprostadil and clopidogrel can improve the curative effect,reduce the degree of inflammatory response,and improve cognitive function in the treatment of AIS,without increasing the incidence of adverse reactions.
