摘要
目的 探讨血清人附睾蛋白4(HE4)及β_(2)微球蛋白(β_(2)-MG)在慢性肾衰竭患者肾纤维化进程中的作用。方法选取2020年1月至2021年8月我院收治的慢性肾衰竭患者210例为观察组,按照临床诊断将患者分为慢性肾衰竭3期组70例、慢性肾衰竭4期组70例、慢性肾衰竭5期组70例,并选取同期于我院进行体检的健康体检者70例为对照组。血清HE4利用罗氏e601电化学发光法检测;血清β_(2)微球蛋白利用日立7600免疫比浊法检测。比较四组研究对象的HE4、β_(2)-MG水平。结果 观察组的HE4、β_(2)-MG表达水平显著高于对照组,差异有统计学意义(P<0.05)。慢性肾衰竭各组间血清HE4及β_(2)-MG水平表达比较,差异有统计学意义(P<0.05)。血清HE4与β_(2)-MG的表达在慢性肾衰竭患者肾纤维化进程中呈正相关(P<0.05)。结论 HE4及β_(2)-MG在慢性肾衰竭患者血清中表达明显升高,且随着慢性肾衰竭进展而逐渐上升。用HE4及β_(2)-MG辅助诊断慢性肾脏病进程和肾脏纤维化发生和发展有重要意义,可作为肾纤维化诊断早期的预测指标,可在临床中推广应用。
Objectives To investigate the role of human epididymal protein 4(HE4)andβ_(2) microglobulin(β_(2)-MG)in the progression of renal fibrosis in patients with chronic renal failure.Methods A total of 210 patients with chronic renal failure admitted to our hospital from January 2020 to August 2021 were selected as the observation group.According to clinical diagnosis,the patients were divided into the group of chronic renal failure stage 3,the group of chronic renal failure stage 4,and the group of chronic renal failure stage 5,with 70 cases in each group.A total of 70 healthy individuals who underwent physical examination in our hospital during the same period were selected as the control group.Serum HE4 was detected using Roche e601 electrochemiluminescence;serumβ_(2) microglobulin was detected using Hitachi 7600 immunoturbidimetry.The expression levels of HE4 andβ_(2)-MG in the four groups were compared.Results The expression levels of HE4 andβ_(2)-MG in the observation group were significantly higher than those in the control group(P<0.05).There were significant differences inserum HE4 andβ_(2)-Mg levels among chronic renal failuregroups(P<0.05).The expression of serum HE4 andβ_(2)-Mg was positively correlated with the progression of renal fibrosis in patients with chronic renal failure(P<0.05).Conclusions The expressions of HE4 andβ_(2)-Mg in serum of patients with chronic renal failure increasedsignificantly,and gradually increased with the progression of chronic renal failure.It is of great importance to use HE4 andβ_(2)-Mg in the diagnosis of chronic kidney disease and the occurrence and development of renal fibrosis,which can be used as an early predictor of renal fibrosis,and can be promoted in clinical application.
