基于数据挖掘的中药治疗结节性痒疹用药规律研究

Study on Medication Law of TCM for the Treatment of Prurigo Nodularis Based on Data Mining

目的分析中药治疗结节性痒疹的用药规律,为临床治疗提供参考。方法检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、中国学术期刊数据库(万方数据)、中国生物医学文献数据库(CBM)建库起至2022年6月30日收录的中药治疗结节性痒疹研究文献。采用知网研学软件进行文献筛选,采用Excel2019软件进行数据规范整理,采用古今医案云平台(V2.3.5)进行用药频次、性味归经分析,聚类分析、关联规则分析、复杂网络分析。结果经筛选,纳入163篇文献,涉及内服方156首(214味药物)、外用处方18首(83味药物)。内服高频药物有当归、白鲜皮、全蝎、刺蒺藜、甘草等,多性寒、温,味多苦、甘,以归肝、心、脾经为主,功效以清热为主;外用高频药物有苦参、蛇床子、白鲜皮、黄柏、白矾等,多性寒、温,味苦、辛,以归肝、脾、心经为主,功效以清热燥湿、杀虫为主。聚类分析可将内服药物聚为6类;外用药物聚为4类。关联规则分析内服药物获得8组药对,提升度最高的为“全蝎-白鲜皮”;外用药物23组药对,支持度最高的为“蛇床子-苦参”。内服药物复杂网络分析得出新处方由托里消毒散、仙方活命饮、白蒺藜散化裁而成。结论中药治疗结节性痒疹内服以清热为主,临证辅以祛风除湿、扶正药物;外用则偏重清热燥湿、杀虫止痒。

Objective To analyze the medication law of TCM in the treatment of prurigo nodularis;To provide references for clinical treatment.Methods The literature on TCM for the treatment of prurigo nodosum was retrieved from China National Knowledge Resource Database(CNKI),Chinese Science and Technology Journal Database(VIP),Chinese Academic Journal Database(Wanfang Data)and China Biology Medicine Disc(CBM)from the establishment of the databases to June 30,2022.CNKI research software was used for literature screening.Microsoft Excel 2019 software was used for data standardization.The ancient and modern medical record cloud platform(V2.3.5)was used for analysis on drug frequency,property and taste,and meridian tropism,and clustering analysis,association rule analysis,and complex network analysis were conducted.Results After screening,163 articles were involved,including 156 oral prescriptions(214 kinds of Chinese materia medica)and 18 external prescriptions(83 kinds of Chinese materia medica).The high-frequency drugs used in oral administration included Angelicae Sinensis Radix,Dictamni Cortex,Scorpio、Fructus Tribuli,Glycyrrhizae Radix et Rhizoma,etc.,which were mostly cold and warm,and the tastes were bitter and sweet,mainly for the liver meridian,heart meridian and spleen meridian,and the main efficacy was to clear heat;the high-frequency drugs for external use included Sophorae Flavescentis Radix,Cnidii Fructus,Dictamni Cortex,Phellodendri Chinensis Cortex,Alumen,etc.,which were mainly cold and warm,and the tastes were bitter and pungent,mainly for liver meridian,spleen meridian and heart meridian,and the efficacy was mainly to clear heat,dry dampness and kill insects.By clustering analysis,the drugs of oral administration could be grouped into 6 categories,and drugs for external use could be grouped into 4 categories.According to association rule analysis,8 medicinal pairs were obtained by drugs of oral administration,among which“Scorpio-Dictamni Cortex”was the most improved;a total of 23 groups of medicinal pairs were obtained for external use,among which“Cnidii Fructus-Sophorae Flavescentis Radix”had the highest support.The complex network analysis of drugs of oral administration showed that the new prescription was composed of Tuoli Xiaodu Powder,combined with Xianfang Huoming Decoction and Baijili Powder.Conclusion In the treatment of prurigo nodularis,TCM of oral administration focuses on clearing heat,supplemented by dispelling wind dampness,and strengthening the body.For external use,it focuses on clearing heat,drying dampness,killing insects and relieving itching.
