基于数据挖掘的针灸治疗多囊卵巢综合征不孕症取穴规律研究

Study on Acupoint Selection Law of Acupuncture and Moxibustion for the Treatment of Infertility with Polycystic Ovary Syndrome Based on Data Mining

目的分析针灸治疗多囊卵巢综合征(PCOS)不孕症的取穴规律,为临床治疗提供取穴思路。方法检索中国知识资源总库(CNKI)、中文科技期刊数据库(VIP)、中国学术期刊数据库(万方数据)、中国生物医学文献数据库(CBM)、PubMed收录的2012年1月1日-2022年3月1日针灸治疗PCOS不孕症临床研究文献。采用Excel2016建立数据库,采用SPSS Modeler 18.0进行关联规则分析,SPSS Statistics 26进行描述性分析和聚类分析。结果经筛选,共纳入79篇文献,涉及93首针灸处方、75个腧穴,腧穴使用总频次718次,频次最高的腧穴为关元;常选的经脉为任脉、脾经、胃经;取穴部位主要涉及腹部、下肢及背部;特定穴常选交会穴;核心处方为关元-中极-子宫-三阴交-足三里。结论针灸治疗本病遵循“固本培元、调理冲任、气血同治”基本原则,临床取穴以阴经为主养精血,辅以阳经补阳气引领阴气,以取特定穴为主,尤其重视交会穴使用,同时根据生理周期特点分期分经辨证取穴以增强疗效。

Objective To analyze the acupoint selection law of acupuncture and moxibustion for the treatment of polycystic ovary syndrome(PCOS)infertility;To provide the idea of acupoint selection for clinical treatment.Methods The clinical studies on acupuncture and moxibustion for PCOS infertility were retrieved from China Knowledge Resources Database(CNKI),Chinese Science and Technology Journal Database(VIP),China Academic Journal Database(Wanfang Data),China Biomedical Literature Database(CBM)and PubMed from January 1,2012 to March 1,2022.Excel2016 was used to establish the database.SPSS Modeler 18.0 was used for association rule analysis.SPSS Statistics 26 was used for descriptive analysis and clustering analysis.Results A total of 79 articles were included through screening,involving 93 acupuncture prescriptions and 75 acupoints.The total frequency of acupoints used was 718 times,and the acupoint with the highest frequency was Guanyuan(RN04);the commonly selected meridians were Conception Vessel,spleen meridian and stomach meridian;acupoint selection locations mainly involved the abdomen,lower limbs and back;specific acupoints often selected intersection acupoints;the core prescription was Guanyuan(RN04)-Zhongji(RN03)-Zigong(EX-CA1)-Sanyinjiao(SP06)-Zusanli(ST36).Conclusion Acupuncture and moxibustion for the treatment of PCOS infertility follows the basic principles of“strengthening the root and nourishing the original qi,regulating Thoroughfare Vessel and Conception Vessel and treating qi and blood together”.Clinical acupoint selection is based on yin meridian to nourish the essence of blood,supplemented by yang meridian to supplement yang qi to guide yin qi,based on specific acupoints,paying particular attention to the use of intersection acupoints.At the same time,according to the characteristics of the physiological cycle,staging meridian syndrome differentiation is used to select acupoints to enhance the curative effect.
