苦参汤联合窄谱中波紫外线照射治疗寻常型银屑病的疗效观察

Clinical Observation on Efficacy of Kushen Decoction and Narrow-band Ultraviolet Radiation in Treatment of Psoriasis Vulgaris

目的探讨苦参汤联合窄谱中波紫外线(narrow band ultraviolet B,NB-UVB)对寻常型银屑病的治疗效果。方法选取2020年1月—2021年12月盘锦市盘山县人民医院收治的银屑病患者100例随机分为对照组和研究组各50例。对照组采用NB-UVB照射治疗;而观察组在其基础上加用苦参汤,两组均治疗12周。比较两组患者治疗后的中医证候评分、炎症因子、免疫细胞亚群变化情况。结果治疗后,研究组患者各项中医证候评分和总分均低于对照组,差异均有统计学意义(均P<0.05)。研究组白介素-4(IL-4)和白介素-10(IL-10)高于对照组;而干扰素-γ(IFN-γ)、白介素-2(IL-2)、白介素-17(IL-17)和肿瘤坏死因子-α(TNF-α)低于对照组,差异有统计学意义(P<0.05)。研究组CD3+和CD4+高于对照组,而CD8+低于对照组,差异均有统计学意义(均P<0.05)。结论苦参汤联合NB-UVB,能够进一步改善寻常型银屑病患者的症状,调节其体内炎症因子平衡,并且强化T细胞免疫功能,值得临床推广应用。

Objective To investigate the therapeutic effect of Kushen Decoction combined with narrow-band ultraviolet B(NB-UVB)on psoriasis vulgaris.Methods A total of 100 patients with psoriasis admitted to Panshan County People’s Hospital hospital from January 2020 to December 2021 were randomly divided into a control group and a study group with 50 patients each.The control group was treated with NB-UVB irradiationand the observation group adopted Kushen Decoction on the base of NB-UVB.Both groups were treated for 12 weeks.The changes in traditional Chinese medicine syndrome scores,inflammatory factors,and immune cell subsets of the two groups after treatment were compared.Results After treatment,the scores and total scores of various traditional Chinese medicine syndromes in the study group were significantly lower than those in the control group(P<0.05).The levels of IL-4 and IL-10 in the study group were significantly higher than those in the control group.And interferon-γ,Interleukin-2,Interleukin-17,and tumor necrosis factor⁃αin the study group were significantly lower than those in the control group(P<0.05).CD3+and CD4+in the study group were significantly higher than those in the control group,while CD8+was significantly lower than those in the control group(P<0.05).Conclusion Kushen decoction combined with NB-UVB can further improve the symptoms of patients with psoriasis vulgaris,regulate the balance of inflammatory factors in the body,and strengthen T cell immune function,which is worthy of clinical promotion and application.
