健脾益肾增液汤联合常规治疗对高龄慢性功能性便秘患者的临床疗效

Clinical effects of Jianpi Yishen Zengye Decoction combined with conventional treatment on patients with elderly chronic functional constipation

目的考察健脾益肾增液汤联合常规治疗对高龄慢性功能性便秘患者的临床疗效。方法102例患者随机分为对照组和观察组,每组51例,对照组给予常规治疗,观察组在对照组基础上加用健脾益肾增液汤,疗程1个月。检测临床疗效、中医证候评分、肠道神经递质(NO、VIP、NPY)、胃肠功能指标(全胃肠排出率、胃排空率)、肠道菌群(肠杆菌、乳酸杆菌、双歧杆菌)数量、不良反应发生率、复发率变化。结果观察组总有效率高于对照组(P<0.05),复发率更低(P<0.05)。治疗后,2组中医证候评分、肠道神经递质、肠杆菌数量降低(P<0.05),胃肠功能指标及乳酸杆菌、双歧杆菌数量升高(P<0.05),以观察组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论健脾益肾增液汤联合常规治疗可安全有效地调控高龄慢性功能性便秘患者肠神经递质因子表达,增强胃肠功能,改善肠道菌群稳态,降低复发风险。

AIM To investigate the clinical effects of Jianpi Yishen Zengye Decoction combined with conventional treatment on patients with elderly chronic functional constipation.METHODS One hundred and two patients were randomly assigned into control group(51 cases)for 1-month intervention of conventional treatment,and observation group(51 cases)for 1-month intervention of both Jianpi Yishen Zengye Decoction and conventional treatment.The changes in clinical efficacy,TCM syndrome scores,intestinal neurotransmitters(NO,VIP,NPY),gastrointestinal function indices(gastrointestinal emptying rate,gastric evacuation rate),intestinal flora(Enterobacteria,Lactobacillus,Bifidobacterium)counts,incidence of adverse reactions and recurrence rate were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05),along with lower recurrence rate(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome scores,intestinal neurotransmitters,Enterobacteria count(P<0.05),and increased gastrointestinal function indices and Lactobacillus,Bifidobacterium counts(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with elderly chronic functional constipation,Jianpi Yishen Zengye Decoction combined with conventional treatment can safely and effectively regulate enteric neurotransmitter factor expressions,enhance gastrointestinal functions,improve intestinal flora homeostasis,and reduce recurrence risk.
