出院带药流程优化探索与实践

Exploration and practice of optimization of discharge medication process

目的优化出院带药流程,改善患者就医体验。方法运用PDCA循环法,开展优化出院带药流程活动。首先梳理出院带药流程,找出导致患者出院带药时间长的关键环节和真因,然后针对真因制订对策并实施,最后通过活动前后100例患者平均出院带药时间等指标对本次活动效果进行评价。结果针对患者出院带药时间长的3个真因“手续繁琐”、“患者取药时间集中”、“药师调配时间长”,采取“送药到病区”出院带药新模式,实现了患者“零跑路”;加强系统信息化,让带药更便捷;优化路线、专人专岗,缩短了药品调配时间;加强“医护患药”沟通,实现了全院推广;床旁用药教育,完成了科学指导患者用药。出院带药时间由活动前的75.6 min缩短为活动后的24 min。结论PDCA循环法能持续优化患者出院带药流程,缩短患者出院带药时间,有助于提升药学服务品质,改善患者就医体验。

Objective To optimize the discharge medication process and to improve the patients’medical experience.Methods PDCA cycle method was used to carry out the activities of optimizing discharge medication process.Firstly,key links and real reasons that affected the long time of taking medicine after discharge were finded out by combing the discharge medication process.Then formulated countermeasures for those real reasons were implemented.Finally,the activity was evaluated by the average discharge medication time of 100 patients before and after the activity.Results According to three real reasons of“complicated procedures”、“taking medicines at the same time”、“long time for drug preparation”,the new mode of delivering medicine to the ward was taken,and it realized that patients took medicine in the ward.By strengthening system informatization it was easier for patients to take medicine after discharge.By optimizing the route and assigning special personnel,the time of drug dispensing was shortened.By communication between pharmacists and doctors,nurses,patients,the new mode was promoted in the whole hospital.Bedsides medication education,clinical pharmacists scientifically guide patients to take medicine.The discharge medication time was shortened from 75.6 min(before PDCA)to 24 min(after PDCA).Conclusion PDCA circulation method can continuously optimize the discharge medication process of patients,and shorten the time of discharge medication effectively.This help our hospital to improve the quality of pharmaceutical care and improve medical experience of patients.
