兔管状肠外瘘模型的建立

摘要 目的旨在构建兔管状肠外瘘动物模型。方法采用随机数表法将12只雄性新西兰大白兔分为两组:实验组8只,开腹后于回肠末端沿长轴造成一0.5 cm切口并留置T管,T管自腹壁斜形切口穿出后固定于腹壁外侧;对照组4只,行剖腹探查手术。术中、术后给予抗生素预防感染,术后禁食3 d,不禁水。记录各组存活率、手术时间、评估窦道形成情况、14 d后检测血清指标以及大体形态变化,同时,取瘘口周围组织观察其组织学的变化。结果对照组和实验组的存活率分别为100%、85.7%。实验组的手术时间为(36.9±3.8)min,实验组和对照组术后14 d的血清总蛋白、血清白蛋白及血糖差异无统计学意义(P>0.05)。实验组腹壁瘘口已形成与消化道相通的稳定窦道,肠管与腹膜紧密相连,窦道周围组织存在有明显的炎症反应。结论可成功构建管状肠外瘘兔动物模型。 Objective Intestinal fistula is a common and serious complication after surgery.Due to the lack of animal models for tubular fistula,preclinical animal experiments are difficult to conduct.The aim of this study was to construct an animal model of tu⁃bular intestinal fistula in rabbits.Methods The random number table method was used to divide twelve male New Zealand White rabbits into two groups.Eight rabbits in the experimental group were incised at the end of the ileum along the long axis of the ileum for 0.5 cm and left with a T tube,which was fixed on the outside of the abdominal wall after passing through an oblique incision.In the control group with 4 rabbits,just a laparotomy was performed.Intraoperative and postoperative antibiotics were given to prevent infec⁃tion.Postoperative fasting had been performed for 3 days.The survival rate,operation time,assessment of fistula formation,detection of serum indexes and gross morphological changes after 14 days were recorded for two groups,while the histological changes of the tis⁃sues around the fistula were taken and observed.Results The survival rate was 100%in the control group and 85.7%in the experi⁃mental group.The operation time of the experimental group was 36.9±3.8 min.There were no significant differences in total serum pro⁃tein,serum albumin and blood glucose between the experimental and control groups 14 days after the operation.In the experimental group,the abdominal wall fistula had formed a stable sinus tract of the digestive tract,the intestinal tube was closely connected to the peritoneum,and there was a significant inflammatory reaction in the tissues surrounding the fistula.Conclusion The rabbit animal model of tubular intestinal fistula is successfully constructed via this protocol.
