摘要 基于以资源为基础的相对价值比率(resource-based relative value scale,RBRVS)的绩效薪酬系统与疾病诊断相关分组(diagnosis related groups,DRGs)/病种分值法(big data diagnosis-intervention packet,DIP)相结合的临床绩效考核评价方法的研究,建立与医院高质量发展相匹配的精益绩效管理体系,医院对医务人员工作技术难度、责任风险、疾病复杂程度、治疗花费时间等临床工作强度和个人能力进行评价,使结果更客观和公平,体现优绩优酬,从而调动临床医务人员的积极性。医院以医疗质量为抓手,从注重科室业绩指标转变为以注重医疗质量、运营管理、医院持续发展和满意度评价和社会效益为根本,加强医院内涵建设,提高医院的绩效管理水平,为医务人员的医疗服务评价、学科发展、人才发展提供科学合理依据,整合基于RBRVS和DRGs/DIP并重的医疗质量管理和成本控费相结合的绩效管理体系,以提高工作效率、提升医务人员工作积极性、改善医院收支结构,从而实现公立医院科学化、精细化、低成本、高效率的管理目标。 Based on the research of clinical performance evaluation method combining RBRVS(resource-based relative value scale)and DRGs(diagnosis related groups)/DIP(big data diagnosis-intervention packet),establish lean performance management system matching with the high quality development of hospital,the evaluation results of medical staff's clinical work and personal ability are more objective and fair in the aspects of technical difficulty,risk of responsibility,complexity of disease,time spent in treatment,etc.It reflects the good performance and good pay,optimizes the proportion of disease structure,and arouses the enthusiasm of clinical medical staff.Take the medical quality as the key point,from pay attention to the department performance index to pay attention to the medical quality,the operation management,the hospital sustainable development and the satisfaction degree appraisal and the social benefit as the foundation,strengthen the hospital connotation construction,to improve the level of hospital performance management,to provide scientific and reasonable basis for the medical service evaluation,discipline development and talent development of medical personnel,integrate the performance management system based on RBRVS and DRGS/DIP,which combines medical quality management with cost control,in order to improve work efficiency,motivation of medical staff and structure of hospital income and expenditure,the scientific,fine,low-cost and high-efficiency management object of public hospital can be realized.
