基于氧化应激探讨毫火针治疗亚临床期早发性卵巢功能不全的应用

A discussion on the clinical application of filiform-fire needle therapy in treating subclinical premature ovarian insufficiency based on oxidative stress

早发性卵巢功能不全逐渐成为妇科的疑难病症。亚临床期早发性卵巢功能不全的诊治旨在将治疗窗口提前,对潜在的早发性卵巢功能不全进行有效干预,进一步减少早发性卵巢功能不全对女性身心健康的严重影响。西医一般针对早发性卵巢功能不全较为严重阶段予对症治疗,存在较大的局限。中医提倡“治未病”,在疾病初期通过恢复“肾-天癸-冲任-胞宫”生殖轴的平衡状态从根本上改善卵巢功能。既往我们只针对某一种疾病去论述相关疗法的靶器官效应及机制,而时常忽略了疗法本身存在的特异性作用途径。火针疗法存在特殊的抗氧化应激的作用途径和机制,已在治疗白癜风、银屑病、骨关节炎等疾病中取得良好的疗效。目前关于火针治疗早发性卵巢功能不全的研究甚少。文章结合毫火针理论与临床研究,介绍毫火针治疗因氧化应激引起的病理性卵泡数量减少和因卵母细胞质量下降导致的卵巢功能不全。

Premature ovarian insufficiency(POI)has gradually become a difficult disease in gynecology.The diagnosis and treatment of subclinical POI aims to advance the treatment window,effectively intervene the potential POI,and further reduce the serious impact of POI on women’s physical and mental health.Symptomatic treatment is generally given in the severe stage of POI in Western medicine,which has great limitations.TCM advocates the theory of preventive treatment of disease,and fundamentally improves ovarian function by restoring the balance of the reproductive axis of kidney-Tiangui(天癸)-Chongren(冲任)-uterus in the early stage of disease.In the past,we only discussed the target organ effects and action mechanism of related therapy for a certain disease,and often ignored the specific action pathway of the therapy itself.Fire needle therapy has a special pathway and action mechanism of anti-oxidative stress,and has achieved good results in the treatment of vitiligo,psoriasis,osteoarthritis and other diseases.At present,there are few studies on treating POI by fire needle therapy.Based on the filiform-fire needle theory and clinical research,treating pathological follicular decrease caused by oxidative stress and ovarian insufficiency caused by the decline of oocyte quality by filiform-fire needle therapy is introduced in the paper.
