内镜下黏膜剥离术联合白苓参甘汤治疗胃黏膜内癌的效果

Efficacy of Endoscopic Mucosal Dissection Combined with Bailing Shengan Decoction in the Treatment of Gastric Intramucosal Carcinoma

目的探讨内镜下黏膜剥离术(ESD)联合白苓参甘汤治疗胃黏膜内癌的效果及对白细胞介素-8(IL-8)、血管内皮细胞生长因子(VEGF)的影响。方法选取2021年11月至2022年11月商丘市第三人民医院消化内科收治的76例胃黏膜内癌患者,按随机数字表法分为对照组和联合组,各38例,对照组接受ESD治疗,联合组接受ESD联合白苓参甘汤治疗。对比两组疗效、不良反应、治疗前后中医证候积分、血清IL-8、VEGF水平。结果治疗8周后联合组治疗有效率高于对照组(P<0.05);经8周治疗后,两组患者中医主症积分和次症积分及血清IL-8、VEGF水平均降低,联合组低于对照组(P<0.05);联合组不良反应总发生率低于对照组(P<0.05)。结论ESD联合白苓参甘汤治疗胃黏膜内癌有助于改善患者证候积分,降低血清IL-8、VEGF水平及不良反应发生率,提高疗效。

Objective To investigate the efficacy of endoscopic mucosal dissection(ESD)combined with Bailing Shengan decoction in the treatment of gastric intramucosal carcinoma and the effects on interleukin-8(IL-8)and vascular endothelial growth factor(VEGF).Methods Seventy-six patients with gastric intramucosal carcinoma admitted to the Department of Gastroenterology of the Third People’s Hospital of Shangqiu from November 2021 to November 2022 were selected and divided into a control group and a combined group,with 38 cases each group,according to the random number table method.The control group received ESD and the combined group received ESD combined with Bailing Shengan decoction.The efficacy,adverse reactions,TCM syndrome scores,serum IL-8 and VEGF levels before and after treatment were compared between the two groups.Results Treatment efficiency were higher in the combined group than in the control group after 8 weeks of treatment(P<0.05).After 8 weeks of treatment,the TCM primary and secondary symptom scores and serum IL-8 and VEGF levels were reduced in both groups,with the combined group being lower than the control group(P<0.05).The overall incidence of adverse reactions was lower in the combined group than in the control group(P<0.05).Conclusion ESD combined with Bailing Shengan decoction for the treatment of gastric intramucosal carcinoma can help improve the patient’s evidence score,reduce serum IL-8 and VEGF levels and the incidence of adverse effects,and improve the efficacy.
