Construction of perioperative nursing quality evaluation index system for thoracic endovascular aortic repair of Stanford type B aortic dissection

目的:构建Stanford B型主动脉夹层胸主动脉腔内修复术(TEVAR)围术期护理质量评价指标体系,旨在为医疗机构Stanford B型主动脉夹层TEVAR围术期的专科护理质量评价提供依据。方法:以结构-过程-结果三维质量评价模式为理论框架,通过半结构式访谈并采用德尔菲函询法进行2轮专家函询,结合层次分析法确定Stanford B型主动脉夹层TEVAR围术期护理质量评价指标及其权重。结果:2轮函询问卷的回收率均为100%,专家的权威系数分别为0.891和0.901,肯德尔和谐系数分别为0.163和0.231。经过2轮函询,最终确定包括3个一级指标、8个二级指标、54个三级指标的Stanford B型主动脉夹层TEVAR围术期护理质量评价指标体系。结论:构建的Stanford B型主动脉夹层TEVAR围术期护理质量评价指标体系有助于改善心血管外科护理服务质量,优化医护、护患关系,也可为专科医疗护理质量的优化和持续改进提供参考。

Objective:To construct the perioperative nursing quality evaluation index system for thoracic endovascular aortic repair(TEVAR)of Stanford type B aortic dissection,and to provide a basis for the evaluation of specialized nursing quality in the perioperative period of Stanford type B aortic dissection endovascular repair of medical institutions.Methods:The perioperative nursing quality evaluation index and weights for TEVAR of Stanford type B aortic dissection were determined based on the“structure-process-result”three-dimensional quality evaluation model as the theoretical framework,through semi-structured interviews and Delphi correspondence method for 2 rounds of expert correspondence and the analytic hierarchy process.Results:The recovery rate of the 2 rounds of questionnaires was 100%,the authority coefficient of experts were 0.891 and 0.901,and Kendall's W were 0.163 and 0.231,respectively.The perioperative nursing quality evaluation index system for TEVAR of Stanford type B aortic dissection which including 3 first-level indicators,8 second-level indicators and 54 third-level indicators was finally determined after 2 rounds of correspondence.Conclusion:The perioperative nursing quality evaluation index and weights for TEVAR of Stanford type B aortic dissection constructed in this study can help improve the quality of nursing service in cardiovascular surgery,optimize the relationship between doctors and nurses,and provide reference for the optimization and continuous improvement of the quality of specialized medical care.
