血管内皮细胞生长因子表达变化对肝癌合并冠心病患者预后的评估价值研究

Evaluation Value of Vascular Endothelial Cell Growth Factor Expression Changes in Patients with CHD

目的探讨血管内皮细胞生长因子(VEGF)表达与肝癌合并冠心病患者预后的相关性。方法回顾性分析90例肝癌合并冠心病患者的临床资料。所有患者均采集癌组织及癌旁组织,采用免疫组化法检测癌组织及癌旁组织中VEGF表达;比较不同VEGF表达患者临床病理特征间的差异;随访3年,统计90例患者存活情况,比较不同VEGF表达患者存活率。结果癌组织VEGF阳性表达率高于癌旁组织,差异有统计学意义(P<0.05);肿瘤直径≥5 cm、分化程度低分化、肿瘤分期Ⅲ期、有淋巴结转移、冠脉狭窄程度重度狭窄患者VEGF阳性表达率高于肿瘤直径<5 cm、分化程度中高分化、肿瘤分期Ⅰ~Ⅱ期、无淋巴结转移、冠脉狭窄程度轻中度狭窄患者,差异有统计学意义(P<0.05);随访3年,90例患者中存活55例,存活率为61.11%(55/90);VEGF阳性表达组存活率[54.29%(38/70)]低于VEGF阴性表达组[85.00%(17/20)],差异有统计学意义(P<0.05)。结论VEGF在肝癌合并冠心病患者中表达水平较高,且肿瘤越大、分期越高、分化程度越低则VEGF表达水平越高,且易出现淋巴结转移,预后较差。

Objective To explore the correlation between vascular endothelial cell growth factor(VEGF)expression and the prognosis of patients with liver cancer and coronary heart disease,so as to guide the clinical treatment work.Methods Clinical data of 90 patients with HCC were also retrospectively analyzed.All cancer and adjacent tissues were collected for immunohistochemistry to detect VEGF expression in cancer and adjacent tissues,comparing the differences between clinical characteristics of patients with different VEGF expression and comparing different 90 patients and the survival rate of patients with different VEGF expression.Results The positive VEGF was higher than adjacent tissues,The difference was statistically significant(P<0.05);Patients with 5cm VEGF,low differentiation,tumor stage,lymph node metastasis,and severe coronary stenosis were higher than tumor diameter<5cm,moderate to high differentiation,tumor stageⅠ-stageⅡ,no lymph node metastasis,mild to moderate coronary stenosis,The difference was statistically significant(P<0.05);Follow-up visits for 3 years,of the 90 patients,55 patients survived,The survival rate was 61.11%(55/90);The survival rate of the VEGF positive expression group[54.29%(38/70)]was lower than that of the VEGF negative expression group[85.00%(17/20)],the difference was statistically significant(P<0.05).Conclusion VEGF is highly expressed in patients with HCC,and larger tumor,higher stage and lower differentiation have higher VEGF expression level,and is prone to lymph node metastasis and poor prognosis.
